目的比较不同喂养方式对足月新生儿血清瘦素水平的影响。方法选择河北医科大学第一医院产科出生的健康足月新生儿66例,其中母乳喂养儿35例,人工喂养儿31例。在66例新生儿出生第3、7、28天采集母乳2mL于洁净试管中;于新生儿出生第7天,抽取其清晨空腹静脉血2mL,留取血清标本装洁净试管,均置于-70℃冰箱保存。配方乳采用雀巢力多精奶粉,按1匙奶粉加30mL水的比例冲调,水温为50℃。采用放射免疫法对足月新生儿血清、母乳及配方乳中瘦素水平进行测定。结果1.健康足月儿66例出生第3、7、28天母乳瘦素水平分别为(1.31±0.67)μg·L-1、(1.33±0.14)μg·L-1、(1.26±0.98)μg·L-1,不同时间母乳瘦素水平比较差异无统计学意义(F=1.78,P>0.05)。配方乳中未检测到瘦素。2.母乳喂养组新生儿出生第7天血清瘦素水平为(3.38±1.54)μg·L-1,人工喂养组新生儿出生第7天血清瘦素水平为(1.91±0.32)μg·L-1,二组血清瘦素水平比较差异有统计学意义(t=3.176,P<0.05)。结论母乳中含有一定水平的瘦素,是新生儿血中瘦素的重要来源。母乳喂养有利于维持新生儿期血中较高的瘦素水平,对新生儿生长发育及能量代谢有重要作用。
Objective To compare the effects of different feeding methods on serum leptin levels in term neonates. Methods 66 healthy full-term newborns were born in Obstetrics and Gynecology of the First Hospital of Hebei Medical University, including 35 cases of breastfeeding and 31 cases of artificial feeding. Breast milk 2mL was collected on the 3rd, 7th and 28th day of 66 newborns in a clean test tube. On the 7th day after birth, 2mL fresh fasting venous blood was collected and the serum samples were collected to clean the test tubes and placed in -70 ℃ refrigerator to save. Formula milk using Nestle force more refined milk, milk powder by 1 spoon plus 30mL water ratio of the charge, the water temperature is 50 ℃. Radioimmunoassay was used to determine the level of leptin in full-term newborn serum, breast milk and formula milk. RESULTS: The breast-fed leptin levels of 66 healthy full-term infants were (1.31 ± 0.67) μg · L-1, (1.33 ± 0.14) μg · L-1, (1.26 ± 0.98) μg · L-1. There was no significant difference in the serum leptin levels at different time points (F = 1.78, P> 0.05). Leptin was not detected in formula. The level of serum leptin was (3.38 ± 1.54) μg · L-1 on the 7th day after birth in breastfeeding group, the level of serum leptin on the 7th day after birth was (1.91 ± 0.32) μg · L- 1, the serum leptin levels in the two groups were significantly different (t = 3.176, P <0.05). Conclusion Breast milk contains a certain level of leptin, which is an important source of leptin in neonates. Breastfeeding is conducive to maintaining high blood levels of leptin during neonatal period, and plays an important role in neonatal growth and energy metabolism.
