目的:研究胸外按压联合物理增加外周血管阻力法(PCPVR)对心跳骤停家猪复苏血流动力学的影响。方法:通过电致颤法建立8分钟室颤家猪模型,16头动物随机分为胸外按压组(对照组)和胸外按压联合PCPVR组(PCPVR组)。两组动物均使用机械按压,而PCPVR组复苏开始动物的四肢由远及近紧密缠绕弹性止血带以此增加外周血管阻力。复苏5分钟后予150J双向波电除颤。复苏过程中连续记录冠状动脉灌注压和颈动脉血流。结果:复苏过程中使用胸外按压联合PCPVR显著改善不同时间点的冠状动脉灌注压及颈动脉血流量。两组动物全部成功复苏。结论:胸外按压联合PCPVR显著改善心跳骤停家猪心肺复苏过程中的血流动力学。
OBJECTIVE: To study the effect of chest compressions combined with physical augmentation of peripheral vascular resistance (PCPVR) on hemodynamics in revoked cardiac arrest in pigs. Methods: Eight-minute domestic pig model of VF was established by electrostimulation. Sixteen animals were randomly divided into chest compression group (control group) and PCPVR group (PCPVR group). Mechanical compression was used in both groups, whereas peripheral vascular resistance was increased in the limbs of the animals beginning with the PCPVR group resuscitated by far and near tight elastic tourniquets. After 5 minutes of recovery to 150J two-way wave defibrillation. Coronary perfusion pressure and carotid artery blood flow were recorded continuously during resuscitation. Results: Thoracic compression and PCPVR during resuscitation significantly improved coronary perfusion pressure and carotid artery blood flow at different time points. All two groups of animals recovered successfully. Conclusions: Thorax compression combined with PCPVR significantly improves hemodynamics during cardiopulmonary resuscitation in spontaneous cardiac arrest in pigs.
