目的 探讨免疫印迹法在不同自身免疫性表皮下大疱病 (SABD)患者基底膜带自身抗体 (BMZ抗体 )分布情况及其在SABD诊断和鉴别诊断中的应用价值。方法 以热分离法制备的人真皮、表皮提取物作为抗原 ,免疫印迹法检测 97例SABD患者血清中BMZ抗体与基底膜抗原结合情况。结果 不同SABD患者血清中存在结合不同真皮、表皮抗原的IgG型和 (或 )IgA型BMZ抗体。大疱性类天疱疮患者血清中IgG和 (或 )IgA型BMZ抗体主要与相对分子质量为 2 30 0 0 0、180 0 0 0、16 5 0 0 0、130 0 0 0、115 0 0 0、970 0 0的表皮多肽结合 ;线性IgA大疱病患者血清中以结合 180 0 0 0、115 0 0 0、970 0 0表皮多肽的IgA型BMZ抗体为主 ;获得性大疱性表皮松解症患者血清中IgG型和IgA型BMZ抗体特异性结合真皮提取物中 2 90 0 0 0多肽 ;大疱性系统性红斑狼疮患者血清中以结合 2 90 0 0 0真皮多肽和16 5 0 0 0、130 0 0 0、970 0 0表皮多肽的IgG型BMZ抗体为主。结论 不同SABD患者血清中BMZ抗体存在异质性 ,免疫印迹法检测表皮下大疱病BMZ抗体阳性率较高 ,可用于该类疾病的辅助诊断和鉴别诊断。
Objective To investigate the distribution of autoantibodies (BMZ antibodies) in basilar membrane of patients with autoimmune subcutaneous subcapsular vesicle disease (SABD) by immunoblotting and its value in the diagnosis and differential diagnosis of SABD. Methods Human dermis and epidermal extracts prepared by thermal isolation method were used as antigen to detect the binding of BMZ antibody to basement membrane antigen in serum of 97 SABD patients by immunoblotting. Results There were IgG and (or) IgA-type BMZ antibodies in different sera of patients with SABD that had different dermal and epidermal antigens. Conclusions The serum levels of BMZ antibodies in different SABD patients are heterogeneous. The positive rate of BMZ antibodies detected by immunoblotting in subepidermal vesicular disease is high, which can be used for the diagnosis and differential diagnosis of these diseases.
