目的 :探讨急诊超声引导下经皮经肝穿刺胆管引流术(PTCD)治疗合并感染性休克的急性梗阻性化脓性胆管炎(AOSC)的疗效及安全性。方法 :对我院68例急诊PTCD治疗的合并感染性休克的AOSC患者的疗效及并发症进行回顾性分析。结果 :68例患者超声引导下PTCD穿刺成功率为100%,无严重并发症发生。62例(91.2%)患者PTCD置管后2-7d感染性休克症状消失,梗阻症状明显好转,胆红素明显下降。6例(8.8%)患者住院期间死亡。其中22例患者在病情稳定后根据梗阻病因行手术治疗后疗效满意;不能手术的40例患者带管生存,带管时间1~18个月,平均8个月。结论 :对合并感染性休克的AOSC患者进行急诊PTCD治疗不仅能及时退黄,同时有助于休克的改善,并且安全可行。
Objective: To investigate the efficacy and safety of emergency ultrasound-guided percutaneous transhepatic biliary drainage (PTCD) in the treatment of acute obstructive suppurative cholangitis (AOSC) complicated with septic shock. Methods: A retrospective analysis was performed on the efficacy and complications of 68 patients with AOSC complicated with septic shock treated with PTCD in our hospital. Results: The success rate of PTCD puncture in 68 patients was 100% without serious complications. Sixty-two patients (91.2%) had symptoms of septic shock disappeared 2-7 days after PTCD catheterization, obstructive symptoms were significantly improved, and bilirubin was significantly decreased. Six patients (8.8%) died during hospitalization. Twenty-two patients were satisfied with the surgical treatment according to the obstructive disease after the disease was stable. The 40 patients with inoperable surgery survived with laparotomy for 1 to 18 months with an average of 8 months. Conclusion: Emergency PTCD treatment of AOSC patients with septic shock can not only get rid of yellow in time, but also help to improve the shock and is safe and feasible.
