目的 研究关节镜手术治疗成人外侧盘状半月板损伤的预后及影像学评价.方法 选择2008年9月-2015年6月我院骨科收治的96例外侧盘状半月板损伤患者,所有患者均于关节镜下行手术治疗,且术后适时开始肌肉力量训练及关节活动训练,观察患者的疗效、术后膝关节功能的改善情况及MRI影像学评价结果.结果 所有患者均随访至术后1年,无感染、撕裂、复发等并发症发生.术后患者膝关节伸直和屈曲活动能力明显增强,与术前比较差异有统计学意义(P<0.05).术后Lysholm膝关节功能评分显著升高,与治疗前比较差异有统计学意义(P<0.05).术后患者各项生活质量评分均明显改善,抑郁及焦虑状态好转,与治疗前比较差异有统计学意义(P<0.05).术后MRI检查显示:采用外侧盘状半月板成形及缝合术治疗的患者中,优(MRI冠状面残留半月板宽度>5mm)27例,优良率达90.0%.结论 采用关节镜手术治疗成人外侧盘状半月板损伤不仅最大程度保留了半月板功能,而且获得了理想的影像学评价结果及确切的膝关节功能改善,该术式适合临床推广应用.“,”Objective To study the prognosis and imaging evaluation of arthroscopic surgery in the treatment of adult lateral discoid meniscus injury.Methods Ninety-six patients with lateral discoid meniscus injury treated in the Affiliated Hospital of North China University of Science and Technology from September 2008 to June 2015 were selected.All patients underwent surgical treatment under arthroscopy,followed with the training of muscle strength training and joint exercise to observe the efficacy of the patients,the improvement of postoperative knee joint function and MRI imaging evaluation results.Results All patients were followed up to 1 years after the operation.No complications,such as infection,tearing and recurrence,were found.The ability of knee joint extension and flexion activity increased significantly after operation,and the difference was statistically significant (P<0.05).The score of Lysholm knee joint function was significantly higher after operation,and the difference was statistically significant compared with that before treatment (P <0.05).The scores of the life quality of the patients were obviously improved,and the depression and anxiety were improved,and the difference was statistically significant (P<0.05).Postoperative MRI examination showed that in the patients treated with lateral discoid meniscus formation and suture,excellent (MRI coronal residual meniscus width>Smm) in 27 cases,the excellent and good rate was 90%.Conclusion The arthroscopic surgery for adult discoid lateral meniscus injury not only preserves the function of meniscus,but also achieves ideal imaging evaluation results and accurate knee function improvement.This method is suitable for clinical application.
