本文对30例慢性阻塞性肺疾病(COPD)合并感染的患者,在合并使用氟哌酸和茶喘平、环丙氟哌酸和茶喘平、氟嗪酸和茶喘平前后对茶碱血清稳态谷浓度、稳态峰浓度、消除半衰期、清除率及药时曲线下面积等药代动力学参数的变化进行比较观察。结果发现氟哌酸加茶喘平、环丙氟哌酸加茶喘平、氟嗪酸加茶喘平后茶碱稳态谷浓度分别增加16.6%、21%和10.5%;稳态峰浓度分别增加14.4%、16.1%和7.6%;消除半衰期分别延长16.7%、20.5%和7.7%;药时曲线下面积分别增大19.6%、22.4%和5.7%;消除率分别下降19.4%、25.8%和1.44%。提示环丙氟哌酸,氟哌酸均降低茶碱消除率,使血清茶碱浓度增高。两者合用时应监测血药浓度,适当调整剂量。而氟嗪酸对茶碱药代动力学影响甚少,与茶碱合用是较安全的。
In this paper, 30 patients with chronic obstructive pulmonary disease (COPD) infection, before and after the combined use of norfloxacin and tea asthma, ciprofloxacin and Chachuan Ping, Steady-state trough concentration, steady-state peak concentration, elimination half-life, clearance rate and area under the curve after pharmacokinetic parameters were observed and compared. The results showed that norfloxacin plus Chachuanping, Ciprofloxacin and Chachuan Ping, ofloxacin and Chacha ping theophylline steady state trough concentrations increased by 16.6%, 21% and 10.5%; steady-state peak concentrations were Increased by 14.4%, 16.1% and 7.6% respectively; the elimination half-lives were prolonged by 16.7%, 20.5% and 7.7% respectively; the area under the drug-hour curve increased by 19.6%, 22.4% and 5.7% respectively; the elimination rates decreased by 19.4% and 25.8% 1.44%. Tip ciprofloxacin, norfloxacin both reduce theophylline elimination rate, the serum theophylline concentration increased. The two should be monitored when combined plasma concentration, the appropriate dose adjustment. And triflum acid on the theophylline pharmacokinetics little effect, combined with theophylline is more secure.
