Objective To explore the effect of modern communication platform used in the follow-up health education of hypertensive patients discharged from hospital and to evaluate their effectiveness and feasibility of promotion. Methods From January 2013 to December 2014, 220 patients with primary hypertension in our hospital were randomly divided into new group and conventional group, with 110 patients in each group. The new group used modern communication platform for follow-up health education, Group only before discharge for health education guidance. Patients were followed up at 3, 6 and 12 months after hospital discharge, and their blood pressure was measured. The difference of two follow-up education methods was evaluated. Results The patients in the new group were better able to master the knowledge related to hypertension and were significantly better than the conventional group in taking medication and receiving healthy lifestyle (P <0.05). The fluctuation of blood pressure was significantly better than that of the conventional group (P <0.05) The 12-month follow-up rate of patients was 99.1%, much higher than that of the conventional group (89.1%, P <0.05). Conclusion The effect of follow-up health education on hypertensive patients through modern communication platform is better than conventional group in follow-up rate, which has certain promotion value.
