目的探讨腹壁横竖切口对再次剖宫产术的影响。方法选择我院2009年1月至2011年12月行二次剖宫产术患者80例,将其分成两组:其中前次手术腹壁是竖切口为A组(n=40),前次手术腹壁是横切口为B组(n=40),比较两种腹壁切口再次手术腹壁及盆腔粘连程度,出血量,开腹时间及总手术时间。结果腹壁竖切口组在开腹时间,手术总时间上与横切口比较P<0.01,手术出血量,P<0.05,差异有统计学意义;横切口组再次手术粘连发生率较腹壁竖切口组高,P<0.05,差异有统计学意义。结论腹壁横切口对于初次手术的患者来说有手术时间短,缝合简单,术后恢复快,切口美观的优点,但对于再次手术的患者来说,腹壁粘连较竖切口重,进腹困难,导致手术时间长,损伤大。对有再次妊娠需要的患者,尽可能行腹壁竖切口术式。
Objective To investigate the effect of transverse abdominal incision on cesarean section again. Methods Eighty patients undergoing secondary cesarean section from January 2009 to December 2011 in our hospital were divided into two groups: group A (n = 40) The abdominal wall was transverse incision for the B group (n = 40). The abdominal wall and pelvic adhesions, blood loss, laparotomy and total operation time were compared between the two incisions. Results The abdominal wall vertical incision group in open time, the total operation time compared with the transverse incision P <0.01, the amount of bleeding, P <0.05, the difference was statistically significant; transverse incision reoperation adhesion rate than the vertical abdominal wall group , P <0.05, the difference was statistically significant. Conclusion The abdominal transverse incision has the advantages of short operation time, simple suturing, quick postoperative recovery and beautiful incision for the patients undergoing initial surgery. However, for patients undergoing reoperation, the abdominal wall adhesions are heavier than the vertical incision and have difficulty in advancing abdomen. Surgery for a long time, great damage. For patients who need pregnancy again, as much as possible abdominal vertical incision.
