目的:探讨CD45RA和CD45RO抗原在白血病及其亚型中的表达和分布及其在白血病鉴别诊断中的应用价值。方法:用间接免疫荧光法检测104例白血病患者骨髓或外周血细胞CD45RA和CD45RO抗原及其它免疫标记的表达。其中急性淋巴细胞白血病(ALL)33例;急性髓细胞白血病(AML)62例;慢性B淋巴细胞白血病(B-CLL)9例。结果:CD45抗原的表达呈现四种形式:CD45RA+/RO-、CD45RA-/RO+、CD45RA+/RO+和CD45RA-/RO-。在淋巴细胞白血病不同类型之间,CD45亚类表达形式明显不同,87%B-ALL呈CD45RA+/RO-表型;70%T-ALL为CD45RA-/RO+;78%B-CLL以CD45RA+/RO+为特征。CD45亚类抗原在AML细胞上的表达较为异质,四种形式的表达率分别为56%,18%,16%和10%,且与AMLFAB亚型之间无明显相关性。除CD14+AML易表达CD45RA+/RO+外,其它髓系相关抗原与CD45亚类的表达形式无相关性。结论:CD45RA/RO抗原可为B-ALL与T-ALL,ALL与B-CLL间的鉴别诊断提供一种补充手段,但对AMLFAB亚型?
Objective: To investigate the expression and distribution of CD45RA and CD45RO antigens in leukemia and its subtypes and their clinical value in the differential diagnosis of leukemia. Methods: The expression of CD45RA and CD45RO antigens and other immune markers in bone marrow or peripheral blood of 104 patients with leukemia were detected by indirect immunofluorescence. Among them, 33 cases were acute lymphoblastic leukemia (ALL), 62 cases were acute myeloid leukemia (AML) and 9 cases were chronic B-lymphoblastic leukemia (B-CLL). Results: CD45 antigens were expressed in four forms: CD45RA + / RO-, CD45RA- / RO +, CD45RA + / RO + and CD45RA- / RO-. CD45RA + / RO- phenotypes were found in 87% of B-ALL patients; CD45RA- / RO + in 70% of T-ALL patients; CD45RA + / RO + in 78% of B-CLL patients for different types of lymphocytic leukemia For the characteristics. The expression of CD45 subclass antigen on AML cells was more heterogeneous. The expression rates of the four forms were 56%, 18%, 16% and 10%, respectively. There was no significant correlation between the expression of CD45 subtype and AMLFAB subtype. Except CD14RA + CD45RA + / RO +, there was no correlation between other myeloid related antigens and the expression of CD45 subclasses. Conclusion: CD45RA / RO antigen can provide a complementary method for the differential diagnosis between B-ALL and T-ALL, ALL and B-CLL.
