Objective To report a case of acute myeloid leukemia (M3) with acute cerebral infarction as the first symptom and to analyze its clinical features and improve neurologists’ understanding of the disease. Methods One case of acute myeloid leukemia patients with acute cerebral infarction as the first symptom was analyzed retrospectively. Results Acute cerebral infarction as the first symptom of leukemia is rare, the pathogenesis is complicated. Patients with diffuse intravascular coagulation are more dangerous and progress faster. Conclusions The acute myeloid leukemia M3 type, which is the first symptom of acute cerebral infarction, is very rare. Misdiagnosis, missed diagnosis, early diagnosis and early treatment are very important for the prognosis of patients.
