目的:探讨拇指桡侧指掌侧固有动脉穿支蒂第一掌骨桡背侧动脉皮瓣修复拇指软组织缺损的临床疗效。方法:自2017年1月至2019年5月,我们共收治拇指软组织缺损患者7例,缺损面积为3.0 cm×2.0 cm~5.0 cm×2.0 cm,根据创面大小及形状设计拇指桡侧指掌侧固有动脉穿支蒂第一掌骨桡背侧动脉皮瓣,转移修复拇指软组织缺损创面。皮瓣供区直接缝合或游离植皮修复。结果:本组7例皮瓣全部存活,随访时间为5~12个月,平均10.1个月,皮瓣外形、色泽、质地与拇指皮肤接近,皮瓣感觉恢复至Sn 2 3例,Sn 3+ 4例。根据手指总主动活动度评定患指功能:优4例,良3例。n 结论:拇指桡侧指掌侧固有动脉穿支蒂第一掌骨桡背侧动脉皮瓣操作简单,不损伤主干血管,可缝合神经重建修复区域感觉,是拇指软组织缺损修复的较好方法之一。“,”Objective:To investigate the clinical efficacy of the first dorsal radial metacarpal artery flap pedicled with the perforator of the palmar proper artery of the radial thumb for repair of thumb soft tissue defects.Methods:From January 2017 to May 2019, 7 cases of thumb soft tissue defects were treated. The defect area ranged from 3.0 cm×2.0 cm to 5.0 cm×2.0 cm. According to the size and shape of the defects, the first dorsal radial metacarpal artery flap pedicled with the perforator of the palmar proper artery of the radial thumb was designed and transferred to repair the thumb soft tissue defects. The donor area of flap was repaired by direct suture or free skin graft.Results:All the flaps survived. The follow-up time was 5 to 12 months with an average of 10.1 months. The shape, color and texture of the flaps were similar to those of the thumb skin. The sensation of the flaps recovered to Sn 2 in 3 cases and Sn 3+ in 4 cases. According to the total active motion of fingers, 4 cases were excellent and 3 cases were good.n Conclusion:The first dorsal radial metacarpal artery flap pedicled with the perforator of the palmar proper artery of the radial thumb is easy to operate, does not damage the main blood vessels, and can be sutured to reconstruct the regional sensation, which is one of the better methods to repair the thumb soft tissue defects.
