目的 研究乳腺肿块能量多普勒超声造影前后血流信号变化和特征。方法 51例乳腺肿块患者 (2 8例良性 ,2 3例恶性 ) ,选用Levovist作为超声造影剂 ,观察造影前后肿块内的血流形态 ,计算机辅助定量测定肿块彩色像素密度及造影后能量多普勒血流信号到达峰值时间。结果 造影后恶性组血流信号增强效果比良性组明显 ,不规则型、穿入型、分支型血流形态在恶性组中分别为 69.6 %、73 .9%、82 .6 % ,在良性组中分别为 2 1 .4 %、32 .1 %、2 8.6 % ,良性组与恶性组比较 ,差异有显著性意义 (P <0 .0 5)。恶性组彩色像素密度增加比良性组明显 ,开始增强时间、到达峰值时间比良性组短 (P <0 .0 5)。结论 Levovist能有效增强乳腺肿块的能量多普勒血流信号 ,有助于进一步观察肿瘤血管形态 ,对乳腺肿瘤的鉴别诊断有帮助
Objective To study the changes and characteristics of blood flow signals before and after Doppler sonography in breast masses. Methods Fifty-one patients with breast masses (28 benign and 23 malignant) were enrolled in this study. Levovist was selected as ultrasound contrast agent to observe the blood flow pattern in the tumor before and after contrast. The computer-aided quantitative determination of mass density of the tumor and post-contrast energy Doppler Blood flow signal reaches peak time. Results Compared with the benign group, the enhancement of blood flow signal in the malignant group after angiography was obvious. The irregular, penetrating and branching blood flow patterns in the malignant group were 69.6%, 73.9% and 82.6% respectively. In the benign group (21.4%, 32.1%, 22.6% respectively). There was significant difference between benign group and malignant group (P <0.05). Malignant group of color pixel density increased significantly more than the benign group, began to enhance the time to reach the peak time shorter than the benign group (P <0.05). Conclusion Levovist can effectively enhance the energy Doppler flow signal in breast lumps, which is helpful for further observation of tumor vessel morphology and is helpful for the differential diagnosis of breast tumors
