目的研究经阴道超声检查对异位妊娠患者的诊断价值。方法纳入青岛经济技术开发区第一人民医院2014年9月至2016年9月期间收治的128例异位妊娠患者作为研究对象,所有患者按入院顺序分为两组。经腹组43例采用经腹部超声检查进行诊断,经阴组85例采用经阴道超声检查进行诊断。观察两种检查方式对异位妊娠的诊断率以及对不同类型异位妊娠的诊断情况。结果经阴组、经腹组患者对异位妊娠确诊率分别为97.53%、79.07%,经阴组对异位妊娠确诊率明显更高(P<0.05)。经阴道超声检查共诊断异位妊娠患者81例,其中输卵管妊娠79例(96.67%),卵巢妊娠1例(1.23%),宫角妊娠1例(1.23%)。未破裂型4例(6.67%);破裂型71例(87.65%);陈旧型6例(7.41%)。两种检查方式在衰减包块型输卵管妊娠、混合包块型输卵管妊娠、实性包块型输卵管妊娠以及漂浮型输卵管妊娠上有明显差异(P<0.05);且经阴组检查各异位妊娠类型诊断率明显高于经腹组(P<0.05)。结论相对于经腹部超声检查而言经阴道超声检查对异位妊娠诊断率和不同类型判断更准确。
Objective To study the value of transvaginal ultrasonography in the diagnosis of ectopic pregnancy. Methods Totally 128 ectopic pregnancy patients admitted to the First People’s Hospital of Qingdao Economic and Technological Development Zone from September 2014 to September 2016 were enrolled in this study. All patients were divided into two groups according to hospital admission order. Forty-three cases of transabdominal group were diagnosed by trans-abdominal ultrasonography and 85 cases of transvaginal ultrasonography. Observe the diagnostic rate of two kinds of examination for ectopic pregnancy and diagnosis of different types of ectopic pregnancy. Results The diagnosis rate of ectopic pregnancy in transvaginal group and transabdominal group were 97.53% and 79.07%, respectively. The diagnosis rate of ectopic pregnancy was significantly higher in the perineum group (P <0.05). Totally 81 cases of ectopic pregnancy were diagnosed by transvaginal ultrasonography, including 79 cases of tubal pregnancy (96.67%), 1 case of ovarian pregnancy (1.23%) and 1 case of cornual pregnancy (1.23%). 4 cases (6.67%) were unruptured, 71 cases (87.65%) were ruptured and 6 (7.41%) were old. There were significant differences between the two methods in the attenuation of tubal pregnancy, mixed mass tubal pregnancy, solid mass tubal pregnancy and floating tubal pregnancy (P <0.05) Type of diagnosis was significantly higher than the transabdominal group (P <0.05). Conclusions Transvaginal ultrasonography is more accurate for the diagnosis of ectopic pregnancy and for different types of transabdominal ultrasonography.
