目的探讨检测血清中EB病毒NA1-IgA和VCA-IgA抗体在鼻咽癌诊断中的价值。方法采用ELISA法检测185例健康人群和93例鼻咽癌患者血清EB病毒NA1-IgA和VCA-IgA抗体,应用灵敏度、特异度、准确性和阴、阳性预测值等参数来评估它们在诊断鼻咽癌中的价值。结果单独检测一种抗体和联合检测两种抗体时EB病毒抗体的灵敏度、特异度、阳性预测值和准确性如下:NA1-IgA分别为:89.1%、94.6%、90.3%、92.8%,VCA-IgA分别为:87.1%、93.5%、88.6%、91.3%,联合检测两种抗体分别为:86.0%、96.8%、93.9%、93.2%。结论EBNA1-IgA与EBVCA-IgA两种抗体联合检测时可显著提高检测的特异度、阳性预测值和准确性,同时在检测过程中可以起到互补作用,对鼻咽癌早期血清学诊断和筛查有重要的价值。
Objective To investigate the value of detection of serum Epstein-Barr virus NA1-IgA and VCA-IgA antibodies in the diagnosis of nasopharyngeal carcinoma. Methods The serum Epstein-Barr virus NA1-IgA and VCA-IgA antibodies in 185 healthy people and 93 patients with nasopharyngeal carcinoma were detected by ELISA. The sensitivity, specificity, accuracy, negative predictive value, and positive predictive value were used to evaluate their diagnostic value The value of pharyngeal cancer. Results The sensitivity, specificity, positive predictive value and accuracy of Epstein-Barr virus antibodies were as follows: NA1-IgA: 89.1%, 94.6%, 90.3%, 92.8% IgA were 87.1%, 93.5%, 88.6% and 91.3% respectively. The combined detection of two antibodies were 86.0%, 96.8%, 93.9% and 93.2% respectively. Conclusion The combined detection of EBNA1-IgA and EBVCA-IgA antibody can significantly improve the specificity, positive predictive value and accuracy of the detection, and can play a complementary role in the detection of nasopharyngeal carcinoma early serological diagnosis and screening Check the important value.
