血液中脂类是作为脂蛋白而存在的,可分为以食物性脂肪为主并在肠粘膜上形成的乳糜微粒(CM)以及主要在肝脏里制造成的极低密度脂蛋白:(VLDL)、高密度脂蛋白(HDL),还有VLDL在血液中经代谢形成的中间密度脂蛋白(IDL)和低密度脂蛋白(LD-L)等五种(世界卫生组织分类法)。CM、VLDL、IDL、LDL有促进动脉硬化的作用,而HDL有预防动脉硬化的作用。清晨空腹血清中CM、VLDL、IDL、LDL中无论哪一种增高都是高脂血症(高脂蛋白血症)。清晨空腹时查到CM则为Ⅰ型高脂血症,如果只有LDL增加则为Ⅱ_a型高脂血症,若IDL和VLDL都增加则为Ⅱ_b型高脂血症,若VLDL和LDL增加,脂蛋白电泳中前β脂蛋白与β脂蛋白没有分离则为Ⅲ型高脂血症(broadβ病)如仅VLDL增加则为Ⅳ型高脂血症,
