目的:研究七味白术散联合胰岛素泵短期强化干预治疗脾虚型妊娠期糖尿病患者的临床疗效及对母婴结局的影响。方法:将92例妊娠期糖尿病患者随机分为治疗组和对照组各46例。对照组采用内装优泌林R的便携式胰岛素泵对妊娠期糖尿病患者注入胰岛素进行短期强化干预治疗,治疗组在采取胰岛素泵短期强化干预治疗的基础上加服七味白术散,两组均以7 d为1个疗程,试验时间为2个疗程。观察两组患者治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白、空腹胰岛素、胰岛素抵抗指数及血脂4项,并对两组患者妊娠结局进行统计分析。结果:两组各项指标治疗后均优于治疗前(P<0.01或P<0.05);治疗组患者空腹血糖、餐后2 h血糖、糖化血红蛋白、空腹胰岛素、胰岛素抵抗指数、甘油三酯、高密度脂蛋白及总胆固醇指数治疗后疗效均优于对照组(P<0.05);治疗组对母婴结局的积极影响明显优于对照组(P<0.05),并减少了剖宫产率,降低了子痫前期、胎膜早破、早产、新生儿呼吸窘迫、巨大儿等并发症发生率。结论:七味白术散联合胰岛素泵短期强化干预治疗脾虚型妊娠期糖尿病患者具有较好临床疗效,并且对母婴结局有积极的影响。
Objective: To study the clinical effect of Qiweibaizhu San combined with short-term intensive insulin pump intervention on gestational diabetes of spleen deficiency type and its effect on maternal and infant outcome. Methods: 92 cases of gestational diabetes mellitus were randomly divided into treatment group and control group, 46 cases each. In the control group, insulin pump was injected into the gestational diabetes mellitus for short-term intensive intervention. Results: The fasting blood glucose, postprandial blood glucose, glycosylated hemoglobin, fasting insulin, insulin resistance index, triglyceride, triglyceride, triglyceride, HDL and total cholesterol index after treatment were better than the control group (P <0.05); the positive effect of the treatment group on maternal and infant outcomes was significantly better than the control group (P <0.05), and reduce the cesarean section rate, Reduced the incidence of preeclampsia, premature rupture of membranes, premature birth, neonatal respiratory distress, giant children and other complications. Conclusion: Qiweibaizhifan combined with short-term intensive insulin pump intervention in the treatment of gestational diabetes in patients with spleen deficiency has a good clinical efficacy, and the maternal and child outcomes have a positive impact.
