目的分析扩张型心肌病(DCM)患者心功能不全与室性心律失常的相关性。方法DCM患者135例,行超声心动图、和24-h动态心电图监测,检测血浆B型利钠肽(BNP)。按纽约心脏病功能(NYHA)心功能分级和左室射血分数(LVEF)分组,比较室性心律失常的发生情况。结果DCM患者NYHA心功能分级与血浆BNP水平呈正相关(rs=0.851,P<0.05),与LVEF呈负相关(rs=-0.681,P<0.05)。随着心功能分级、BNP水平增高以及LVEF的降低,DCM患者严重室性心律失常的发生率增高(P<0.05)。结论 DCM患者心功能损害程度与严重室性心律失常的发生密切相关。联合应用血浆BNP和LVEF检测能预测DCM患者发生室性心律失常的风险。
Objective To analyze the correlation between ventricular dysfunction and ventricular arrhythmia in patients with dilated cardiomyopathy (DCM). Methods A total of 135 DCM patients underwent echocardiography and 24-h Holter monitoring. Plasma BNP was detected. According to New York Heart Association (NYHA) cardiac function classification and left ventricular ejection fraction (LVEF) grouping, compare the incidence of ventricular arrhythmias. Results There was a positive correlation between NYHA functional class and plasma BNP level in patients with DCM (rs = 0.851, P <0.05), but negatively correlated with LVEF (rs = -0.681, P <0.05). The incidence of severe ventricular arrhythmias was significantly increased in patients with DCM (P <0.05) as cardiac function was graded, BNP levels were increased, and LVEF was decreased. Conclusion The degree of cardiac dysfunction in patients with DCM is closely related to the occurrence of severe ventricular arrhythmia. The combination of plasma BNP and LVEF can predict the risk of ventricular arrhythmias in patients with DCM.
