目的探讨米索前列醇联合芬太尼及丙泊酚用于无痛人工流产的麻醉效果。方法收集十堰市妇幼保健院妇产科2011年3月至2012年3月间入院并需进行人工流产手术的妇女81例。使用随机法将上述所有患者分为两组,即对照组40例与观察组41例。两组患者均行医院常规人工流产手术,对照组患者术中采用丙泊酚行常规麻醉。而治疗组患者术中则采用给予米索前列醇联合芬太尼及丙泊酚联合用药进行麻醉。针对两组患者术中麻醉效果、清醒时间、苏醒时VAS评分以及手术出血量等资料进行对比分析。结果两组患者手术中麻醉效果对比差异,其观察组显效及总有效率显著高于对照组,且结果差异(P<0.05)具有统计学意义。而两组患者清醒时间、苏醒时VAS评分以及手术出血量对比差异,观察组患者也明显优于对照组患者,且结果差异(P<0.05)具有统计学意义。结论米索前列醇联合芬太尼及丙泊酚用于无痛人工流产具有显著的麻醉效果,且可以有效减轻患者术中疼痛及手术出血量,提高苏醒速度及苏醒质量。故此方法安全有效,值得临床推广。
Objective To investigate the anesthetic effect of misoprostol combined with fentanyl and propofol for painless induced abortion. Methods A total of 81 women admitted to the Department of Obstetrics and Gynecology, MCH from March 2011 to March 2012 were enrolled in this study. All patients were divided into two groups according to the randomized method: control group (40 cases) and observation group (41 cases). The two groups of patients underwent routine abortion in the hospital, while patients in the control group received propofol anesthesia during operation. The patients in the treatment group were given intraoperative misoprostol combined with fentanyl and propofol for anesthesia. According to the data of intraoperative anesthesia, awake time, VAS score at awakening, and the amount of bleeding during operation, the data of two groups were compared. Results There was significant difference between the two groups in the anesthetic effect during operation. The effective rate and total effective rate in the observation group were significantly higher than those in the control group, and the difference was statistically significant (P <0.05). The awake time of the two groups, the VAS score on awakening and the amount of bleeding after surgery were significantly different from those of the control group, and the difference was statistically significant (P <0.05). Conclusion Misoprostol combined with fentanyl and propofol have significant anesthetic effect in painless induced abortion, and can effectively reduce intraoperative pain and blood loss, improve wakefulness and wakefulness. Therefore, this method is safe and effective, it is worth clinical promotion.
