目的:探讨白介素-6(IL-6)、白介素-8(IL-8)与胎膜早破、新生儿预后的关系。方法:收集2011年1月~2014年1月的160例胎膜早破孕妇作为病例组;随机抽取同期正常孕妇112例作为对照组。两组孕妇入院后即抽取静脉血提取血清,采用ELISA法检测血清中IL-6、IL-8的含量,统计分析病例组和对照组感染绒毛膜羊膜炎的几率及新生儿预后的发生率,采用western blot测定胎盘组织中IL-6、IL-8含量。结果:病例组血清中IL-6、IL-8的含量明显高于对照组,差异有统计学意义;病例组感染绒毛膜羊膜炎的几率明显高于对照组,差异有统计学意义;研究发现IL-6浓度在第1段(21~25 pg/ml)相对于第2段(25~30 pg/ml)、第3段(30~35 pg/ml)新生儿预后差异具有统计学意义(P<0.05);IL-8浓度在第1段(25~30 pg/ml)相对于第2段(30~35 pg/ml)、第3段(35~40 pg/ml)新生儿预后差异具有统计学意义(P<0.05)。结论:IL-6、IL-8可能全程参与妊娠过程,因此联合监测IL-6、IL-8的含量对于预测胎膜早破新生儿的预后有一定的作用,尤其在降低新生儿死亡率、预防远期并发症等方面有一定价值。
Objective: To investigate the relationship between interleukin-6 (IL-6), interleukin-8 (IL-8) and the prognosis of premature rupture of membranes and neonates. Methods: One hundred and sixty pregnant women with premature rupture of membranes from January 2011 to January 2014 were selected as the case group. One hundred and twelve normal pregnant women were randomly selected as the control group. Serum levels of IL-6 and IL-8 were detected by ELISA. The incidences of chorioamnionitis and neonatal prognosis in the case and control groups were statistically analyzed. Western blot was used to determine the content of IL-6 and IL-8 in placenta. Results: The serum levels of IL-6 and IL-8 in case group were significantly higher than those in control group, the difference was statistically significant. The incidence of chorioamnionitis in case group was significantly higher than that in control group, and the difference was statistically significant. The difference in the prognosis of neonates with IL-6 in stage 1 (21-25 pg / ml) vs stage 2 (25-30 pg / ml) and stage 3 (30-35 pg / ml) was statistically significant P <0.05). The difference of IL-8 concentration in stage 1 (25-30 pg / ml) vs stage 2 (30-35 pg / ml) and stage 3 Statistically significant (P <0.05). IL-6 and IL-8 may participate in the process of pregnancy. Therefore, the combined monitoring of the levels of IL-6 and IL-8 plays a role in predicting the prognosis of premature rupture of membranes, especially in reducing neonatal mortality, Prevention of long-term complications have some value.
