目的:研究动脉内血压脉动指数与冠状动脉病变程度的相关性,探讨影响血压脉动指数的因素。方法:选择215例拟诊冠心病的患者,经右桡动脉行冠脉造影检查,计算Gensini积分;同步记录升主动脉、肱动脉及桡动脉血压参数,并计算相关脉动指数,结合年龄、血糖、血脂、体重指数及超声心动图等参数,研究动脉内血压脉动指数与冠脉病变程度的相关性;并分析影响动脉内血压脉动指数的因素。结果:单因素相关分析发现:年龄、吸烟、总胆固醇、低密度脂蛋白、中心脉动指数、中心脉压、肱动脉脉压、肱动脉脉动指数、射血分数均与Gensini积分相关(P<0.05),动脉内血压脉动指数主要与年龄和女性呈正相关(均P<0.01)。多因素Logistic回归分析显示:年龄(β=0.414,P=0.01)和肱动脉脉动指数(β=0.287,P=0.04)对冠心病有独立的预测价值。结论:动脉内肱动脉血压脉动指数与冠脉病变的严重程度相关,是冠心病的预测因子。
Objective: To study the correlation between arterial blood pressure pulsation index and coronary artery lesion, and to explore the factors influencing blood pressure pulsation index. Methods: 215 patients with suspected CHD were enrolled in this study. Gensini scores were calculated by right radial artery and coronary angiography. The parameters of ascending aorta, brachial artery and radial artery were recorded simultaneously and the related pulsatility index was calculated. The age, blood glucose , Lipids, body mass index and echocardiography were used to study the correlation between arterial pressure pulsation index and severity of coronary artery disease. The factors affecting arterial blood pressure pulsation index were analyzed. Results: Univariate analysis showed that age, smoking, total cholesterol, low density lipoprotein, central pulsatility index, central pulse pressure, brachial artery pulse pressure, brachial artery pulse index and ejection fraction were all correlated with Gensini score (P <0.05 ), Arterial pulse pressure index mainly with age and women were positively correlated (all P <0.01). Multivariate Logistic regression analysis showed that age (β = 0.414, P = 0.01) and brachial artery pulse index (β = 0.287, P = 0.04) had independent predictive value for CHD. Conclusion: The intra-arterial brachial artery pulse pressure index is related to the severity of coronary artery disease and is a predictor of coronary heart disease.
