目的探讨热休克自体胃癌细胞负载的树突状细胞(DC)疫苗对胃癌术后细胞免疫功能的影响。方法从胃癌患者外周血单个核细胞中诱导DC,并用重组人粒细胞-巨噬细胞集落刺激因子(rh-GM-CSF)和白介素4(rhIL-4)刺激活化,经负载热休克凋亡的自体胃癌细胞制备DC疫苗。将32例胃癌术后化疗患者随机分为DC疫苗治疗组16例,另16例作为对照组,仅作常规化疗。对两组病例治疗前后T细胞亚群及自然杀伤(NK)细胞进行观察比较。结果DC疫苗治疗后外周血CD3+、CD3+CD4+、CD3+CD8+及NK细胞比率较治疗前明显升高(P<0.05),且明显高于对照组化疗后的相应指标(P<0.05)。结论胃癌术后化疗时行热休克凋亡自体胃癌细胞负载的DC疫苗治疗,可改善患者的细胞免疫水平。
Objective To investigate the effect of dendritic cell (DC) vaccine loaded with heat shock autologous gastric cancer cells on cellular immune function after gastric cancer surgery. Methods DCs were induced from peripheral blood mononuclear cells in patients with gastric cancer and stimulated with recombinant human granulocyte-macrophage colony-stimulating factor (rh-GM-CSF) and interleukin 4 (rhIL-4). Heat shock was induced by heat shock. DC vaccine was prepared from autologous gastric cancer cells. Thirty-two patients with postoperative chemotherapy for gastric cancer were randomly divided into two groups: 16 in the DC vaccine group and 16 in the control group. The T cell subsets and natural killer (NK) cells were observed before and after treatment in both groups. Results The percentage of CD3+, CD3+CD4+, CD3+CD8+, and NK cells in peripheral blood was significantly higher after DC vaccine treatment than before treatment (P<0.05), and it was significantly higher than that in control group after chemotherapy (P<0.05). Conclusions Gastric cancer can be improved by the heat shock treatment of autologous gastric cancer cells loaded with DC vaccine.
