目的 探讨同种异体脑膜、巩膜与硅胶加固对变性近视治疗对比。方法位于角膜缘後4mm处,保留鼻侧90°结膜做270°切口,暴露上、外、下直肌及下斜肌,视後巩膜的葡萄肿与视网膜病变范围,采用不同宽度的加固带置于肌肉下方并固定。结果硅胶组(25眼)术後炎症反应较巩膜组(19眼)明显,结膜水肿严重,有2眼发生排斥反应,异体脑膜加固(36眼)较巩膜加固术後结膜反应轻,且发膜加同与脑膜加固组无排斥反应现象。结论 术後超声检查眼轴缩短1~4mm占51%。追踪观察3个月至5年,45例60眼视力不同程度提高,玻璃体混浊明显吸收,眼底灰白色朦胧状得以改善。异体脑膜优于异体巩膜,异体巩膜优于硅胶加固。
Objective To investigate the effects of allogenic meninges, sclera and silicone on the treatment of degenerative myopia. Follow-up observation of 3 months to 5 years, 45 cases of 60 eyes improved in varying degrees of visual acuity, vitreous opacity was significantly absorbed, dim white eye shadow can be improved. Allogeneic meningeal is superior to allogeneic sclera, allogeneic sclera is better than silica gel.
