目的:通过测定妊娠期高血压疾病患者胎盘组织中的基质金属蛋白酶-2和基质金属蛋白酶-9的表达情况,探讨妊娠期高血压疾病发病的病因。方法:选取在汕头大学医学院第一附属医院产科住院足月分娩92例孕妇,正常妊娠40例为对照组,妊娠期高血压疾病患者例52为研究组,其中轻度子痫前期27例,重度子痫前期25例,应用免疫组化法,检测MMP-2和MMP-9在胎盘组织中的表达。结果:①MMP-2在妊娠期高血压疾病患者胎盘组织中的阳性表达与正常妊娠组相比,差异有统计学意义(P<0.05);轻度子痫前期组,重度子痫前期组与正常妊娠3组比较,差异有统计学意义(P<0.05)。重度子痫前期组与轻度子痫前期组比较,尽管胎盘组织MMP-2阳性表达降低,两组间差异无统计学意义(P>0.05)。②MMP-9在妊娠期高血压疾病患者胎盘组织中的阳性表达与正常妊娠组相比,差异有统计学意义(P<0.05);重度子痫前期组与轻度子痫前期组比较,差异有显著性(P<0.05)。结论:MMP-2和MMP-9在妊娠期高血压疾病组患者胎盘组织中表达的降低,导致滋养细胞浸润能力下降,可能与妊娠期高血压疾病发病有关,且与病情轻重呈正相关。
OBJECTIVE: To investigate the expression of matrix metalloproteinase-2 and matrix metalloproteinase-9 in placenta of patients with hypertensive disorder complicating pregnancy and to explore the etiology of hypertensive disorder complicating pregnancy. Methods: Ninety-two pregnant women were delivered in term hospitalization in the First Affiliated Hospital of Shantou University Medical College. Forty cases of normal pregnancy as control group and fifty-two cases of hypertensive disorder complicating pregnancy were selected as study group, including 27 cases of mild preeclampsia, 25 cases of severe preeclampsia, using immunohistochemistry to detect the expression of MMP-2 and MMP-9 in placenta. Results: (1) The positive expression of MMP-2 in placental tissue of patients with hypertensive disorder complicating pregnancy was significantly lower than that of normal pregnancy group (P <0.05). There was significant difference between mild preeclampsia group and severe preeclampsia group There were significant differences in gestational 3 groups (P <0.05). Compared with mild preeclampsia group, the expression of MMP-2 in placental tissue decreased in severe preeclampsia group and there was no significant difference between the two groups (P> 0.05). The positive expression of MMP-9 in placental tissue of patients with hypertensive disorder complicating pregnancy was significantly higher than that of normal pregnancy group (P <0.05). There was significant difference between severe preeclampsia group and mild preeclampsia group Significance (P <0.05). Conclusion: The decreased expression of MMP-2 and MMP-9 in placental tissue of patients with gestational hypertension may result in decreased ability of trophoblast invasion, which may be related to the incidence of hypertensive disorder complicating pregnancy and positively correlated with the severity of the disease.
