目的研究差异性剂量的硝酸甘油与氯吡格雷联合疗法对冠心病心绞痛患者的疗效和预后。方法选择我院收治的86例冠心病心绞痛患者作为研究对象,随机分为对照组和观察组,每组43例。对照组患者给予氯吡格雷药物治疗;观察组患者又分为观察A组(21例)、观察B组(22例),前者采用小剂量硝酸甘油和氯吡格雷联合治疗,后者采用大剂量的硝酸甘油和氯吡格雷联合治疗;比较3组患者的疗效。结果观察组患者临床疗效为优27例,优良率为95.35%,显著高于对照组的26例、60.47%,差异有统计学意义(P<0.05);但观察A组和观察B组患者的疗效优良率差异无统计学意义(P>0.05)。用药后,观察组不良反应发生率为18.60%,与对照组的23.26%差异无统计学意义(P>0.05),且观察A组和观察B组患者的不良反应发生率差异也无统计学意义(P>0.05)。结论硝酸甘油与氯吡格雷联用对冠心病心绞痛患者的疗效更好,用药后不良反应较少。为了进一步减少不良反应发生率,硝酸甘油的使用应结合实际情况,从小剂量开始使用,更安全可靠,实用性强。
Objective To study the effect and prognosis of patients with coronary heart disease and angina pectoris treated with different doses of nitroglycerin and clopidogrel. Methods Eighty-six patients with coronary heart disease and angina pectoris admitted to our hospital were randomly divided into control group and observation group, with 43 cases in each group. Patients in the observation group were divided into observation group A (n = 21) and observation group B (n = 22). The former was treated with combination of low-dose nitroglycerin and clopidogrel, and the latter with high-dose Of nitroglycerin and clopidogrel combined treatment; the efficacy of three groups of patients were compared. Results The clinical curative effect in observation group was excellent in 27 cases, the excellent and good rate was 95.35%, which was significantly higher than that in control group (26 cases, 60.47%), the difference was statistically significant (P <0.05). However, in observation group A and observation group B There was no significant difference between the two groups (P> 0.05). After treatment, the incidence of adverse reactions in the observation group was 18.60%, which was not significantly different from that in the control group (23.26%) (P> 0.05), and there was no significant difference in the incidence of adverse reactions between the observation group A and the observation group B (P> 0.05). Conclusion The combination of nitroglycerin and clopidogrel has a better curative effect on patients with coronary heart disease and angina pectoris, with fewer adverse reactions after treatment. In order to further reduce the incidence of adverse reactions, the use of nitroglycerin should be combined with the actual situation, starting from a small dose, safer and more reliable, practical.
