目的研究内皮素(ET)和一氧化氮(NO)在突发性感音神经性聋的发生、发展及预后中的作用,探讨突聋的发病机制,为临床诊治突聋及判断预后提供理论依据。方法对2002-08~2003-02长沙地区26例突聋患者进行血浆ET和血清一氧化氮代谢产物硝酸盐的含量测定,并同期对18例健康对照者进行测定。结果突聋患者治疗前ET水平明显高于健康对照者,而NO水平明显低于健康对照者(P<0.05);治疗有效组治疗后ET水平明显降低达基本正常水平(P<0.05),而NO水平明显提高,但仍低于健康对照者(P<0.05);治疗有效组治疗前ET水平明显低于无效组,NO明显高于无效组,治疗后亦如此(P<0.05);不同程度耳聋组之间ET和NO水平无显著性差别(P>0.05)。结论血浆ET水平增高和血清NO水平减低在突聋的发生、发展过程中可能起着一定的作用;血浆ET水平增高程度和血清NO水平减低程度与突聋患者的耳聋程度无关,但对判断其预后可能起着重要作用。
Objective To investigate the role of endothelin (ET) and nitric oxide (NO) in the occurrence, development and prognosis of sudden sensorineural hearing loss, and to explore the pathogenesis of sudden deafness and to provide a theoretical basis for clinical diagnosis and treatment of sudden deafness and prognosis . Methods The levels of plasma ET and nitric oxide in serum were measured in 26 patients with sudden deafness in Changsha district from August 2002 to February 2003, and 18 healthy controls were measured in the same period. Results The level of ET in patients with sudden deafness was significantly higher than that in healthy controls (P <0.05), and the level of NO in patients with deafness was significantly lower than that in healthy controls (P <0.05). (P <0.05). The ET level of the effective group before treatment was significantly lower than that of the ineffective group, while the NO level was significantly higher than that of the ineffective group (P <0.05) There was no significant difference between ET and NO levels in deafness group (P> 0.05). Conclusions The increase of plasma ET level and the decrease of serum NO level may play a role in the occurrence and development of sudden deafness. The increase of plasma ET level and the level of serum NO level have nothing to do with the degree of deafness in patients with sudden deafness. However, Prognosis may play an important role.
