目的:采用高甘油三脂钳闭技术比较结构甘油三酯(STG)与长链甘油三酯(LCT)在健康志愿者中的代谢效应,以评估结构脂肪乳剂代谢特征。方法:选择16位健康志愿者进行前瞻性随机交叉对照研究。采用改良的高甘油三酯钳闭技术,维持血浆甘油三酯浓度在4mmol/L水平达180分钟,比较STG和LCT的代谢特征。结果:STG的血浆清除率较LCT快一倍左右,STG组血清总游离脂肪酸、游离甘油及β-羟基丁酸盐浓度明显高于LCT组,血浆脂蛋白脂酶(LPL)活性明显大于LCT组并与所输注的甘油三酯量密切相关。结论:高甘油三酯钳闭模式可重复、有效的比较不同的脂肪乳剂的代谢特征,健康志愿者血浆STG的清除率明显大于LCT,STG的水解速度明显快于LCT,血浆LPL活性增高有利于STG的快速清除
OBJECTIVE: To compare the metabolic effects of structural triglycerides (STG) and long-chain triglycerides (LCT) in healthy volunteers using high triglyceride clamp technique to assess the metabolic profile of structured fat emulsions. Methods: Sixteen healthy volunteers were enrolled in a prospective randomized controlled cross-over study. The modified high triglyceride clamp technique was used to maintain the plasma triglyceride concentration at 4 mmol / L for 180 minutes. The metabolic characteristics of STG and LCT were compared. Results: The plasma clearance rate of STG was almost double that of LCT. The concentrations of total free fatty acids, free glycerol and β-hydroxybutyrate in STG group were significantly higher than those in LCT group. The activity of plasma lipoprotein lipase (LPL) was significantly higher than that of LCT group And is closely related to the amount of triglycerides infused. CONCLUSION: The high triglyceride clamp model can reproducibly and effectively compare the metabolic characteristics of different lipid emulsions.
