目的:探讨钛制弹性髓内钉(TEN)治疗儿童股骨干骨折的疗效。方法:2006年10月~2008年8月共收治65例儿童股骨干骨折患者,分别给予TEN固定和切开复位钢板固定方法。术后酌情外固定,去除外固定后进行功能锻炼。结果:65例患者术后经平均8.5个月(6.5~18.0个月)随访,TEN固定骨折愈合时间平均为10.4周(8~12周);切开复位钢板固定骨折愈合时间平均为14.3周(12~16周)。所有患者患侧膝关节活动度无明显差异。结论:适当使用TEN治疗儿童股骨干骨折,具有损伤小、应力遮挡小、不影响骨骺、骨折愈合好、患肢功能恢复快、并发症少等优点,但应严格掌握TEN直径的选择、预弯的部位和程度、钉尾的处理等要点。
Objective: To investigate the efficacy of titanium elastic nail (TEN) in the treatment of femoral shaft fractures in children. Methods: From October 2006 to August 2008, 65 children with femoral shaft fractures were enrolled. TEN fixation and open reduction and fixation were performed respectively. After surgery, as appropriate, external fixation, removal of external fixation after functional exercise. RESULTS: Totally 65 patients were followed up for an average of 8.5 months (range, 6.5 to 18.0 months). The average healing time of TEN was 10.4 weeks (range, 8-12 weeks). The average healing time of open reduction and plate fixation was 14.3 weeks 12 to 16 weeks). All patients had no significant difference in knee joint activity. Conclusion: The proper use of TEN for the treatment of femoral shaft fractures has the advantages of less damage, less stress occlusion, no effect on the epiphysis, good fracture healing, fast recovery of limb function, fewer complications and so on. TEN diameter selection, The location and extent of nail tail treatment and other points.
