目的探讨有创与无创序贯性机械通气治疗慢性阻塞性肺疾病(COPD)致严重呼吸衰竭的临床应用效果。方法选取2012年8月至2014年2月我院收治的COPD致严重呼吸衰竭患者47例,随机分为两组,常规组患者给予传统气管插管机械通气治疗,试验组患者接受有创与无创序贯性机械通气治疗,分别对两组患者的呼吸机相关性肺炎(VAP)发生率、再插管率、院内病死率等进行比较。结果试验组患者的VAP发生率、有创通气时间、总通气时间以及住院时间均明显少于常规组,差异均有统计学意义(P<0.05),两组患者再插管率和病死率相比,差异无统计学意义(P>0.05)。结论 COPD致严重呼吸衰竭患者进行有创与无创序贯性机械通气治疗,可加快患者病情改善速度,临床疗效更佳。
Objective To investigate the clinical effect of invasive and noninvasive sequential mechanical ventilation in the treatment of severe respiratory failure caused by chronic obstructive pulmonary disease (COPD). Methods Forty-seven patients with severe respiratory failure who were admitted to our hospital from August 2012 to February 2014 were randomly divided into two groups. The patients in the conventional group were given mechanical ventilation for tracheal intubation. The patients in the experimental group received invasive and noninvasive Sequential mechanical ventilation treatment, respectively, the two groups of patients with ventilator-associated pneumonia (VAP) incidence, re-intubation rate, hospital mortality were compared. Results The VAP incidence, invasive ventilation time, total ventilation time and hospitalization time in the experimental group were significantly less than those in the conventional group (P <0.05). The reperfusion rate and mortality in both groups The difference was not statistically significant (P> 0.05). Conclusions The patients with severe respiratory failure in COPD who underwent invasive and noninvasive sequential mechanical ventilation therapy can speed up the patient’s condition improvement and have better clinical efficacy.
