[目的]探讨非肿块型乳腺癌的乳腺全景数字化X线摄影及导管造影特征,以提高非肿块型乳腺癌的诊断水平。[方法]回顾性分析经病理证实的194例非肿块型乳腺癌的乳腺全景数字化摄影X线征象及导管造影表现。[结果]194例非肿块型乳腺癌的乳腺全景数字化摄影X线特征:微小钙化145例(74.7%),腺体密度增高98例(50.5%),局限性结构紊乱124例(63.9%),血管影增粗89例(45.9%),皮肤增厚38例(19.6%),乳头凹陷29例(14.9%),腋下淋巴结转移3例(1.5%)。17例临床表现为乳头溢液的非肿块型乳腺癌行导管造影,导管造影表现为导管走形僵直、导管扩张或不扩张,管壁毛糙、不规整,管腔内不规则充盈缺损,致狭窄、中断。乳腺全景数字化X线摄影及乳腺导管造影诊断非肿块型乳腺癌的准确率为90.7%。[结论]乳腺全景数字化X线摄影诊断非肿块型乳腺癌较优,对于乳头溢液的患者,辅以导管造影,可以明显提高病灶的检出率和诊断准确率。
[Objective] To investigate the characteristics of digital panoramic mammography and ductal angiography in non-mass-type breast cancer in order to improve the diagnosis of non-mass-type breast cancer. [Methods] Nine hundred and ninety - four non - tumorous breast cancers confirmed by pathology were retrospectively analyzed for digital mammography and ductal radiography. [Results] Ninety-four cases of non-mass breast cancer were characterized by X-ray features of digitalized photography of the breast: microcalcification in 145 cases (74.7%), glandular density in 98 cases (50.5%) and confinement disorders in 124 cases (63.9% 89 cases (45.9%) had thickening of blood vessel, 38 cases (19.6%) had thickened skin, 29 cases (14.9%) had nipple depression and 3 cases (1.5%) had axillary lymph node metastasis. 17 cases of clinical manifestations of nipple discharge of non-mass breast cancer catheterization catheter imaging showed rigid catheterization, catheter expansion or non-expansion, the wall rough, irregular, irregular filling of the defect within the lumen, resulting in stenosis , Interrupted. The accuracy of digital mammography and mammography in the diagnosis of non-mass breast cancer was 90.7%. [Conclusion] Digital panoramic radiography for breast cancer is superior to non-mass breast cancer. For patients with nipple discharge supplemented with catheterization, the detection rate and diagnostic accuracy of lesions can be significantly improved.
