采用前瞻性群组研究方法,对963例对象置器后立即B超测量IUD上缘至宫底外缘距离并随访其避孕效果。测得距离平均为14.24±2.02mm,IUD的脱落频数随距离的增加而增大,置器后18个月,距离≤14mm对象组的粗累积脱落率明显低于距离>14mm对象组,距离每增加1mm,其脱落的相对危险增加1.25倍,但距离因素对妊娠率和因症取出率无明显影响。结果显示,把IUD顶端置入距宫底外缘距离≤14mm的区域内,更可能减少脱落,降低脱落的相对危险,提高续存率。提示有可能在B超的纵切观测下为“宫底部位”作出相应的定量概念。
A prospective cohort study was conducted to measure the distance between the upper edge of the IUD and the edge of the uterus and the contraceptive effect of 963 patients immediately after the device was placed. The measured distance averaged 14.24 ± 2.02mm. The frequency of shedding of IUD increased with the increase of distance. After 18 months, the cumulative shedding rate of objects ≤14mm was significantly lower than that of objects> 14mm, Increased 1mm, the relative risk of shedding increased by 1.25 times, but the distance factors on the pregnancy rate and withdrawal rate had no significant effect. The results show that placing the tip of the IUD within a distance of ≤14 mm from the outer edge of the palace floor is more likely to reduce shedding, reduce the relative risk of shedding and improve the survival rate. Suggesting that it is possible to make a corresponding quantitative concept for “uterine fundus” under the B-slice longitudinal observation.
