目的探讨2型糖尿病(T2DM)下肢血管病变(LLA)与血管内皮功能的关系。方法 71例T2DM患者通过彩色多普勒超声确诊的LLA严重程度分为:无LLA 26例、轻度17例、中度15例、重度13例;另选30名健康人为正常对照(NC)组。采用酶联免疫吸附双抗体夹心法,检测代表血管内皮功能的血浆血管性假血友病因子(vWF)和单核细胞趋化蛋白-1(MCP-1)水平。结果血浆MCP-1、vWF水平,糖尿病无LLA组明显高于NC组,有LLA组明显高于无LLA组,以及各LLA亚组间比较差异均有统计学意义(P均<0.01)。各LLA亚组下肢血管评分与血浆MCP-1(r=0.926,P<0.01)、vWF(r=0.942,P<0.01)水平正相关。结论血管内皮功能影响下肢血管病变。
Objective To investigate the relationship between lower extremity vascular lesions (LLA) and vascular endothelial function in type 2 diabetes mellitus (T2DM). Methods The severity of LLA diagnosed by color Doppler ultrasonography in 71 T2DM patients was divided into 26 cases without LLA, 17 with mild, 15 with moderate and 13 with severe. Another 30 healthy subjects were selected as normal control (NC) group . The levels of plasma vWF and MCP-1, which represent the function of vascular endothelium, were detected by enzyme-linked immunosorbent assay (ELISA) double antibody sandwich method. Results The levels of plasma MCP-1, vWF and diabetes in LLA group were significantly higher than those in NC group, significantly higher in LLA group than in non-LLA group, and statistically significant in all LLA subgroups (all P <0.01). Vascular score of lower extremities in each LLA subgroup was positively correlated with plasma MCP-1 (r = 0.926, P <0.01) and vWF (r = 0.942, P <0.01). Conclusion Vascular endothelial function affects lower extremity vascular disease.
