选择 4 0例血压正常的 2型糖尿病患者进行 2 4h动态血压及心电图监测 ,计算 2 4h全部正常心律R -R间期的标准差 (SDNN) ,单位为毫秒 ,并与 30例健康者比较。结果 :糖尿病组白天血压无明显改变 ,夜间舒张压及平均动脉压较对照组升高 (P <0 0 1 )。且其收缩压、舒张压、平均动脉压的夜间下降百分率较对照组明显降低 (均P <0 0 1 ) ,正常昼夜节律消失。取平均动脉压夜间下降百分率与SDNN作相关分析 ,示两者显著正相关。结论 :血压正常的 2型糖尿病患者已存在血压节律的改变 ,且此改变与心率变异显著相关。
Forty normotensive patients with type 2 diabetes mellitus were enrolled in 24-hour ambulatory blood pressure monitoring and electrocardiogram (ECG) monitoring. SDNN of all R-R intervals of 24 hrs were calculated and expressed in milliseconds and compared with 30 healthy controls. Results: There was no significant change of daytime diastolic blood pressure, diastolic blood pressure and mean arterial pressure in diabetic group compared with the control group (P <0.01). And the systolic blood pressure, diastolic blood pressure, mean arterial pressure night decreased percentage was significantly lower than the control group (P <0.01), the normal circadian rhythm disappeared. Correlation analysis of SDNN and percentage of nocturnal decline in mean arterial pressure showed a significant positive correlation between them. CONCLUSIONS: Blood pressure rhythm changes have been observed in normotensive patients with type 2 diabetes, and this change is significantly associated with heart rate variability.
