缺血性脑血管病患者颈动脉粥样硬化斑块稳定性和血清MMP-2、IL-6表达的相关性

目的探讨缺血性脑血管病患者颈动脉粥样硬化斑块稳定性和血清MMP-2、IL-6表达的关系。方法选择在本院住院治疗的缺血性脑血管病患者100例为研究对象(缺血性脑血管病组)。所有研究对象均行颈动脉超声检查,根据斑块的超声检查特征分为稳定斑块组(39例)、不稳定斑块组(27例),斑块性质介于两组之间者为中间组(34例)。另选择同期在我院行健康检查的体检者30例为对照组。采用双抗体夹心酶联免疫吸附试验(ELISA)测定各组患者血清MMP-2、IL-6的水平。结果缺血性脑血管病组血清MMP-2、IL-6水平均较对照组明显增高,差异均有统计学意义(P<0.01)。三组不同斑块稳定性患者的血清MMP-2、IL-6水平比较差异有统计学意义(P<0.01)。经线性相关性分析,血清MMP-2、IL-6水平均与颈动脉斑块易损程度呈正相关(r=0.69、0.64,P<0.01)。对100例缺血性脑血管病患者血清MMP-2与IL-6浓度表达进行Spearman等级相关分析,发现MMP-2水平与IL-6水平呈正相关(r=0.754,P<0.01)。结论缺血性脑血管病患者血清MMP-2、IL-6水平明显升高,与颈动脉斑块易损程度密切相关,且两者共同作用促使缺血性脑血管病发生。 Objective To investigate the relationship between the stability of carotid atherosclerosis plaque and the expression of serum MMP-2 and IL-6 in patients with ischemic cerebrovascular disease. Methods 100 cases of ischemic cerebrovascular disease hospitalized in our hospital were selected as the research object (ischemic cerebrovascular disease group). All subjects underwent ultrasound examination of carotid artery. According to the features of plaque ultrasound examination, they were divided into stable plaque group (n = 39) and unstable plaque group (n = 27). Plaque quality was between two groups Group (34 cases). Another option at the same period in our hospital for physical examination of 30 patients as a control group. The levels of serum MMP-2 and IL-6 in each group were measured by ELISA. Results Serum levels of MMP-2 and IL-6 in ischemic cerebrovascular disease group were significantly higher than those in control group (P <0.01). The levels of serum MMP-2 and IL-6 in three groups with different plaque stability were significantly different (P <0.01). Linear correlation analysis showed that serum MMP-2 and IL-6 levels were positively correlated with carotid artery plaque vulnerability (r = 0.69,0.64, P <0.01). Spearman rank correlation analysis of serum MMP-2 and IL-6 levels in 100 patients with ischemic cerebrovascular disease showed that MMP-2 level was positively correlated with IL-6 level (r = 0.754, P <0.01). Conclusions Serum levels of MMP-2 and IL-6 in patients with ischemic cerebrovascular disease are significantly increased, which are closely related to the vulnerability of carotid plaque, and the combination of both can promote the occurrence of ischemic cerebrovascular disease.
