目的:探讨高原地区2型糖尿病(T2DM)伴周围神经病变患者桡神经的超声特征,为病情评估及治疗提供依据。方法:选取我院34例2型糖尿病患者分为周围神经病变组21例和无周围神经病变组13例,并选择同期17例非糖尿病患者作为对照,所有受检者桡神经行超声检查,比较各组桡神经的左右径、前后内径、截面积和血流情况。结果:糖尿病周围神经病变组较无周围神经病变组桡神经双侧左右径及内径增大,且均较对照组增大(P<0.01)。糖尿病周围神经病变组桡神经超声主要表现为神经束回声减低、筛网状结构模糊和神经外膜增厚、不平整,与毗邻组织分界不清。糖尿病周围神经病变组、无周围神经病变组及对照组桡神经血流信号比较,差异无统计学意义(P>0.05)。结论:糖尿病伴周围神经病变患者沿桡神经沟走行中距体表最近处桡神经的超声特征为神经束回声减低、筛网状结构模糊和神经外膜增厚、不平整,与毗邻组织分界不清,为评估患者的周围神经病变程度以及治疗方案的选择提供形态学依据。
Objective: To investigate the ultrasonographic features of radial nerve in patients with type 2 diabetes mellitus (T2DM) and peripheral neuropathy in the plateau, and to provide basis for evaluation and treatment of the disease. Methods: Twenty-four patients with type 2 diabetes in our hospital were divided into 21 cases of peripheral neuropathy group and 13 cases of peripheral neuropathy group. 17 cases of non-diabetic patients in the same period were selected as control. All patients underwent ultrasound examination of radial nerve. Radial nerve left and right diameter, anteroposterior diameter, cross-sectional area and blood flow in each group. Results: Compared with the control group, the bilateral left and right diameter and the diameter of the radial nerve increased significantly in diabetic peripheral neuropathy group than in the non-peripheral neuropathy group (P <0.01). Diabetic peripheral neuropathy group radial nerve ultrasound mainly for the reduction of the acoustic echo of the beam, the sieve structure fuzzy and epineurial thickening, uneven, and adjacent tissue demarcation is not clear. There was no significant difference in radial nerve blood flow between diabetic peripheral neuropathy group, peripheral neuropathy group and control group (P> 0.05). Conclusion: The characteristics of the radial nerve in the patients with diabetes mellitus and peripheral neuropathy walking along the radial nerve ditch are the following: the acoustic characteristics of the radial nerve are decreased; the meshed structure is fuzzy and the epineurium is thickened and irregular; the border with the adjacent tissue is not Qing, in order to assess the degree of patients with peripheral neuropathy and treatment options to provide a morphological basis.
