背景近年来应变率成像技术评价心脏功能愈加受到临床关注,实时三平面应变率成像技术是新近发展起来的新技术。目的探讨实时三平面应变率成像技术评价原发性高血压患者左心室重构过程中长轴舒张功能变化的临床价值。方法选择原发性高血压患者115例和健康志愿者35例,根据Ganau左心室几何构型划分法,将原发性高血压患者分正常构型(36例)、向心性重构型(24例)、离心性肥厚型(22例)及向心性肥厚型(33例)。应用实时三平面应变率成像技术测定各受检者左心室各壁基底段和中间段舒张早期、舒期晚期峰值应变率(SRe、SRa)。结果SRe在正常人、正常构型、向心性重构型、离心性肥厚型及向心性肥厚型中逐渐减小(P值分别<0.05与0.01),前壁和侧壁SRa正常构型组低于离心性肥厚组(P值分别<0.05与0.01)。结论实时三平面应变率成像技术能够准确评价原发性高血压患者左心室重构进展过程中长轴舒张功能的变化,SRe可作为评价左室局部舒张功能的一个有用而敏感的指标。
Background In recent years, strain rate imaging has become the focus of clinical evaluation of cardiac function. Real-time three-plane strain rate imaging is a newly developed technology. Objective To investigate the clinical value of real-time three-plane strain rate imaging in evaluating long-term diastolic function during left ventricular remodeling in patients with essential hypertension. Methods A total of 115 patients with essential hypertension and 35 healthy volunteers were enrolled in this study. According to the Ganau left ventricular geometry configuration, the patients with essential hypertension were divided into normal control group (36 cases), concentric remodeling group (24 cases) Cases), eccentric hypertrophy (22 cases) and concentric hypertrophy (33 cases). Real time three-plane strain rate imaging was used to measure the peak strain rate (SRe, SRa) of early and late diastolic phase in the basal and middle segments of the left ventricular wall of each subject. Results SRe decreased gradually in normal subjects, normal conformation, concentric remodeling, eccentric hypertrophy and concentric hypertrophy (P <0.05 and 0.01, respectively) In eccentric hypertrophy group (P <0.05 and 0.01 respectively). Conclusion Real-time three-plane strain rate imaging can accurately evaluate the changes of long-axis diastolic function during left ventricular remodeling in patients with essential hypertension. SRe can be used as a useful and sensitive indicator to evaluate left ventricular diastolic function.
