目的探讨先天性十二指肠梗阻(DUO)患儿的产前超声表现与预后的关系。方法收集53例DUO患儿的临床资料、预后及产前超声表现(胃泡及十二指肠的宽径、羊水指数等)。依据随访结局分为A组13例(不良结局组)、B组40例(无不良结局组),比较A、B两组间的产前超声表现。结果 (1)产前胃泡、十二指肠宽径及羊水指数均与孕周呈正相关,胃泡与十二指肠宽径比值在各孕周中较为恒定;(2)A组的胃泡及十二指肠宽径均大于B组,但差异无统计学意义(P>0.05);两组间羊水指数及产前首次发现异常的孕周无明显差异(P>0.05)。结论胃泡、十二指肠宽径及羊水指数等产前超声表现与预后无明显关系。
Objective To investigate the relationship between prenatal ultrasound findings and prognosis in children with congenital duodenal obstruction (DUO). Methods The clinical data, prognosis and prenatal ultrasound findings of 53 cases of DUO were collected (the diameter of the stomach and the duodenum, the amniotic fluid index, etc.). According to the outcome of follow-up, there were 13 patients in group A (adverse outcome group) and 40 patients in group B (no adverse outcome group). The prenatal ultrasound performance was compared between A and B groups. Results (1) Prenatal gastropathy, duodenum wide diameter and amniotic fluid index were positively correlated with gestational age, and the ratio of the diameter of gastric bubble to duodenum was relatively constant in each gestational week; (2) (P> 0.05). There was no significant difference in the amniotic fluid index between the two groups and the first gestational week of prenatal abnormalities (P> 0.05). Conclusions The prenatal ultrasound findings such as gas bubble, duodenal width and amniotic fluid index had no significant relationship with prognosis.
