目的:观察米力农对重症手足口病(HFMD)患儿疗效及对血清中去甲肾上腺素(NE)水平的影响。方法:以100例3岁以下重症手足口病患儿为研究对象,所有病例随机分为2组:对照组和米力农组,2组患儿均给予必要的对症治疗:抗感染、脱水、限制水入量、吸氧、利尿、镇静、止惊等。米力农组以0.5μg/kg-1min-1的速度静脉滴注米力农,连续用药时长不超过72小时,病情稳定后予停用。观察2组治疗前后心率、呼吸、心输出量(CO)、血清去甲肾上腺素变化。结果:治疗72小时后,2组的心率、呼吸降低,心输出量(CO)升高,血清去甲肾上腺素降低,较治疗前好转,且米力农组的好转效果明显优于对照组,差异有统计学意义(P<0.05)。结论:(1)米力农可以有效的改善重症手足口病的心率、呼吸、心输出量,并可降低去甲肾上腺素水平。(2)去甲肾上腺素在重症手足病治疗中可以起到一定评价作用。
Objective: To observe the effect of Milrinone on the therapeutic effect of severe hand-foot-mouth disease (HFMD) in children and the level of norepinephrine (NE) in serum. Methods: A total of 100 infants with severe HFMD under 3 years of age were selected as study subjects. All cases were randomly divided into two groups: control group and milrinone group. Both groups were given the necessary symptomatic treatment: anti-infective, dehydration, Limit water intake, oxygen, diuretic, sedation, only shock and so on. Milrinone at a rate of 0.5μg / kg-1min-1 infusion of milrinone, continuous medication duration of not more than 72 hours, the disease was stable after the disabled. The changes of heart rate, respiration, cardiac output (CO) and serum norepinephrine were observed before and after treatment in two groups. Results: After 72 hours of treatment, the heart rate, respiration, cardiac output (CO) and norepinephrine in the two groups decreased, which was better than before treatment. The improvement of Milrinone group was better than that of the control group, The difference was statistically significant (P <0.05). Conclusion: (1) Milrinone can effectively improve heart rate, respiration and cardiac output of severe HFMD and reduce norepinephrine level. (2) norepinephrine in the treatment of severe chimpanzees can play a role in evaluation.
