目的比较卡马西平和奥卡西平单药治疗儿童额叶癫的疗效。方法收集本院新诊断的额叶癫47例儿童。男29例,女18例;年龄3~12岁;发作频率17~40次.d-1。根据就诊顺序按单双号顺序分成2组:卡马西平组和奥卡西平组,分别给予卡马西平和奥卡西平单药治疗。奥卡西平起始剂量8 mg.kg-1.d-1,每周增加5 mg.kg-1,逐渐加量至15~30 mg.kg-1.d-1,最大剂量40 mg.kg-1.d-1;卡马西平起始剂量5 mg.kg-1.d-1,每周增加5 mg.kg-1,逐渐增加剂量至10~20 mg.kg-1.d-1或其血药质量浓度达6~10 mg.L-1。随访6个月后评价2组疗效及不良反应。采用SPSS10.0软件进行统计学分析。结果共45例完成观察和随访,卡马西平组22例,奥卡西平组23例。卡马西平组13例完全控制发作(59.1%),3例部分控制(13.6%),其中3例随访6个月时EEG检查正常。奥卡西平组9例控制发作(39.1%),4例部分控制(17.4%),其中5例随访6个月时EEG正常。二组完全控制率、总有效率及EEG的改善比较均无统计学差异(Pa>0.05)。结论奥卡西平和卡马西平单药治疗儿童额叶癫均有很好疗效和耐受性。
Objective To compare the curative effect of carbamazepine and oxcarbazepine on children with frontal lobe epilepsy. Methods Forty-seven cases of newly diagnosed frontal lobe epilepsy were collected in our hospital. 29 males and 18 females, aged 3 to 12 years old, seizure frequency of 17 to 40 times.d-1. According to the order of treatment according to the order of single and double numbers, the rats were divided into two groups: carbamazepine group and oxcarbazepine group, which were given carbamazepine and oxcarbazepine monotherapy respectively. The initial dose of oxcarbazepine 8 mg.kg-1.d-1, weekly increase of 5 mg.kg-1, gradually increased to 15 ~ 30 mg.kg-1.d-1, the maximum dose of 40 mg.kg -1.d-1; carbamazepine starting dose of 5 mg.kg-1.d-1, weekly increase of 5 mg.kg-1, and gradually increase the dose to 10 ~ 20 mg.kg-1.d-1 Or its plasma concentration of 6 ~ 10 mg.L-1. After 6 months of follow-up, two groups were evaluated for efficacy and adverse reactions. Using SPSS10.0 software for statistical analysis. Results A total of 45 cases completed the observation and follow-up, 22 cases were in carbamazepine group and 23 cases were in oxcarbazepine group. Thirteen patients in the carbamazepine group had complete control episodes (59.1%) and three patients (13.6%) partially controlled. Three of them were normal at 6 months of follow-up. In the oxcarbazepine group, 9 patients were controlled (39.1%) and 4 patients were partially controlled (17.4%). EEG was normal in 5 patients at 6 months of follow-up. The two groups of complete control rate, total effective rate and improvement of EEG showed no significant difference (Pa> 0.05). Conclusions Both oxcarbazepine and carbamazepine are effective and tolerable for treating frontal lobe epilepsy in children.
