目的分析自慢性化脓性中耳炎患者中耳分泌物分离的西弗射盾子囊霉的实验室检查特征,为少见真菌的诊治提供依据。方法纳入西弗射盾子囊霉相关慢性化脓性中耳炎患者12例。培养其中耳分泌物,观察真菌的菌落特征和镜下特征,采用Vitek 2 Compact全自动微生物分析系统进行菌种鉴定。结果西弗射盾子囊霉在血平板、巧克力平板和沙氏平板等临床常用培养基上均可生长,且在28℃和35℃均生长较好。培养1周后菌落呈奶油色、镶嵌生长,质地硬,周边呈花边样。该菌在科玛嘉念珠菌显色培养基上为中央蓝色、边缘白色的凸起小菌落。镜下特征为孢子和菌丝同时可见,小培养观察时可见菌丝有分隔,真菌孢子呈芽生型、形态卵圆、部分可沿菌丝排列。西弗射盾子囊霉可同化利用许多碳水化合物和常见氨基酸。用Vitek 2 Compact全自动微生物分析系统可鉴定该菌。结论西弗射盾子囊霉是一种非常少见的机会致病性真菌,临床实验室应重视中耳分泌物中此真菌的培养鉴定。
OBJECTIVE: To analyze the laboratory characteristics of Aspergillus Vesicola in isolation of middle ear secretions from patients with chronic suppurative otitis media and to provide basis for the diagnosis and treatment of rare fungi. Methods Twelve patients with chronic suppurative otitis media were included in the Aspergillus foetida. The middle ear secretions were cultured to observe the colonies and microscopic features of the fungi. Vitek 2 Compact automatic microbial analysis system was used to identify the strains. Results Aspergillus oryzae could grow on clinical common medium such as blood plate, chocolate plate and Sabouraud plate, and grew well at 28 ℃ and 35 ℃. After one week of culture, the colony was creamy, with inlaid growth, hard texture and lace around. The bacteria in the Kama Koshi Candida color medium for the central blue, the edge of the white raised small colonies. Microscopic characteristics of the spores and mycelium visible at the same time, observed in small cultures observed mycelial separation, fungal spores were budding, morphological oval, part of the arrangement along the mycelium. Aspergillus foetida assimilates many carbohydrates and common amino acids. The bacteria can be identified using the Vitek 2 Compact automated microbiological analysis system. Conclusion Aspergillus spp. Is a rare opportunistic pathogenic fungus, and clinical laboratories should attach importance to the culture identification of this fungus in the middle ear secretions.
