目的 探讨眼轮匝肌蒂颞区皮瓣的解剖学基础。方法 对 15具成人尸体的 30侧头面部进行肉眼解剖 ,观察眼轮匝肌的形态及血供情况 ,并对 15例活体行颈总动脉数字减影血管造影 (DSA) ,观察颞浅动脉分支与眼轮匝肌营养血管的关系 ,然后将信息输入计算机图像分析仪行图像分析。结果 1眼轮匝肌宽薄 ,分布广泛 ,血液由多个动脉分支供给 ,血运良好 ,位置表浅并以腱膜直接附着于皮肤 ,其中一部分可以被利用 ,使其可能成为肌皮瓣的蒂。 2眼轮匝肌有多条血管在眼睑内形成动脉网 ,此动脉网与供应颞区皮肤的颞浅动脉分支——额支和颧眶动脉相吻合 ,且颧眶动脉位置较恒定 ,平均口径在 1.0 mm以上。 3颞区皮肤质地、颜色、弹性较好。结论 颧眶动脉是眼轮匝肌蒂颞区皮瓣的主要营养血管 ,以眼轮匝肌为蒂的颞区皮瓣可作为修复面部组织缺损的重要皮瓣。

Conclusions The zygomatic orbital artery is the main nutrient vessel of the temporal flap of the orbicularis oculi muscle. The temporal flap pedicled off the orbicularis oculi muscle can be used as an important flap to repair facial tissue defects.
