目的:通过X线头影测量分析,比较反式Twin-Block和Frankel-Ⅲ功能矫治器治疗生长发育期安氏Ⅲ类错的作用机制。方法:选择替牙期、恒牙早期安氏Ⅲ类错患者20例(男9例,女11例),平均年龄11岁2个月,随机分为T、F2组,每组各10例患者,T组采用反式Twin-Block矫治器矫治,F组采用Frankel-Ⅲ矫治器矫治。全天戴用矫治器治疗4~11个月。分别对治疗前后头颅侧位定位片进行分析比较。结果:2组下颌骨位置均后移,稳定在治疗前的下颌后退位处(P>0.05);2组上颌骨长度及突度均增加,F组效果优于T组(P<0.05);F组有明显的牙齿代偿性移动(P<0.01);T组的平均疗程6.5个月,F组为9个月。结论:2种功能矫治器均能有效的使下颌后退矫治生长发育期安氏Ⅲ类错。Frankel-Ⅲ对上颌的作用大于反式Twin-Block,反式Twin-Block的治疗时间明显短于Frankel-Ⅲ。
OBJECTIVE: To compare the mechanism of trans-Twin-Block and Frankel-Ⅲ functional appliance in the treatment of Angle Ⅲ malocclusion during growth and development by X-ray cephalometric analysis. Methods: Twenty patients (9 males and 11 females) with early malocclusion of type III malocclusion in the early stage of permanent dentition were enrolled. The average age was 11 years and 2 months. The patients were randomly divided into T and F2 groups, with 10 cases in each group Patients in group T were treated with trans Twin-Block appliance and in group F were treated with Frankel-III appliance. Wearing appliances for treatment throughout the day 4 to 11 months. Before and after treatment, respectively, the cephalometric analysis of the skull. Results: The mandibular position of the two groups shifted posteriorly and stabilized at the position of mandibular retraction before treatment (P> 0.05). The length and the protrusion of the maxilla increased in both groups, and the effect in group F was better than that in group T (P <0.05). F group had significant tooth movement (P <0.01). The mean duration of treatment in group T was 6.5 months and in group F was 9 months. Conclusion: The two kinds of functional appliances can effectively make the mandibular recumbent treatment of Class Ⅲ malocclusion in growth and development. The effect of Frankel-III on the upper jaw was greater than that of the trans-Twin-Block, and the treatment time of trans-Twin-Block was significantly shorter than that of Frankel-III.
