目的:总结中期妊娠引产出血原因以及护理措施。方法:选取2010年12月至2014年12月在该院产科进行中期引产的孕产妇作为探讨对象,寻找中期妊娠引产出血原因,并探讨不同护理方式对减少产后出血量的影响。结果:200例中期妊娠引产产妇中22例发生出血,出血率为11.0%,其中催产素(18例)引产产后出血2例,出血率为11.1%,米索前列醇和米非司酮(28例)引产后产后出血1例,出血率为3.6%,利凡诺尔(134例)引产后产后出血18例(13.4%),剖宫产取胎(11例)后发生出血1例,出血率为9.1%,水囊(9例)引产产妇产后未发生出血;导致产后出血的原因主要包括宫缩乏力、胎盘胎膜因素以及凝血功能障碍异常;观察组产妇引产后2h和24h出血量显著低于对照组(P<0.05)。结论:中期妊娠产妇引产后出血受到多种因素的影响,其中引发原因主要以胎盘胎膜残余、凝血功能障碍和宫缩乏力为主,针对引产出血原因给予术前心理疏导护理,可明显缩短出血时间和减少出血量。
Objective: To summarize the causes of induction of labor in mid-term pregnancy and nursing measures. Methods: From December 2010 to December 2014, the pregnant women who were induction of labor in the obstetrics department of the hospital from December 2010 to December 2014 were selected as the research object to find out the cause of induction of labor induced by mid-term pregnancy and to explore the effect of different nursing methods on the reduction of postpartum hemorrhage. Results: In the 200 cases of induction of labor in mid-term pregnancy, bleeding occurred in 22 cases and the rate of bleeding was 11.0%. Among them, oxytocin (n = 18) induced bleeding after delivery in 2 cases, bleeding rate was 11.1%, misoprostol and mifepristone ) 1 postpartum hemorrhage after induction of labor, the rate of bleeding was 3.6%, Rivanol (134 cases) induced postpartum hemorrhage in 18 cases (13.4%), cesarean section fetus (11 cases) after bleeding occurred in 1 case, the bleeding rate was 9.1%, water sac (9 cases) induction of labor postpartum bleeding did not occur; cause postpartum hemorrhage mainly include uterine inertia, placental membranes and coagulation disorders; maternal induction of labor 2h and 24h postpartum bleeding was significantly lower than Control group (P <0.05). Conclusion: The bleeding after induction of labor in mid-term pregnancy is affected by many factors, of which the main causes of placental fetal membrane residual disorders, coagulation dysfunction and uterine atony-based, given the cause of bleeding induced preoperative psychological counseling, can significantly reduce bleeding Time and reduce the amount of bleeding.
