目的探讨小儿轮状病毒感染性脓毒症中,中性粒细胞CD11b、IL-6、hs-CRP水平变化及其临床意义。方法分析我院自2009年10月至2012年1月收治的103例轮状病毒肠患儿。根据是否为脓毒症分为脓毒症组51例与非脓毒症组52例。分析比较脓毒症组与非脓毒症组腹泻持续时间、发热、血小板减少、受累脏器情况,以及中性粒细胞CD11b、白细胞介素-6、hs-CRP水平变化。比较轮状病毒肠炎肠外脏器损害患儿,脓毒症组与非脓毒症组的中性粒细胞CD11b、白细胞介素-6、hs-CRP水平变化。结果两组患者发热(5.1,4.7)、血小板减少[45.1%(423/52),13.4%(7/51)]、受累脏器情况比较[54.9%(28/52),32.7%(17/51)],在统计学上具有统计学意义(P<0.05,P<0.005)。脓毒症组与非脓毒症组CD11b(95.6%,80.2%)、IL-6(24.0,71.9)、hs-CRP(18.2,9.9)水平比较,在统计学上差异具有统计学意义(P<0.05)。毒症组与非脓毒症受累脏器患儿CD11b(98.9%,89.4%)、IL-6(33.1,78.7)、hs-CRP(18.4,11.9)水平,在统计学上差异具有统计学意义(P<0.05)。结果 RV肠炎合并脓毒症时发热持续时间长,血小板减少以及受累脏器增多,同时CD11b、IL-6、hs-CRP水平显著升高,可以作为预测脓毒症标准的指标,对指导临床有一定意义。
Objective To investigate the changes of neutrophil CD11b, IL-6 and hs-CRP in children with rotavirus sepsis and its clinical significance. Methods We analyzed 103 cases of rotavirus enteropathy in our hospital from October 2009 to January 2012. According to whether sepsis is divided into sepsis group 51 cases and 52 cases of non-sepsis group. The duration of diarrhea, fever, thrombocytopenia, organ involvement and the levels of neutrophil CD11b, IL-6 and hs-CRP in sepsis group and non-sepsis group were analyzed. To compare the changes of neutrophil CD11b, interleukin-6, hs-CRP in sepsis group and non-sepsis group in children with intestinal organ damage due to rotavirus enteritis. Results The incidence of fever (5.1,4.7), thrombocytopenia (45.1% (423/52), 13.4% (7/51)] in the two groups were significantly higher than those in the control group [54.9% (28/52) vs 32.7% (17/51) 51)], was statistically significant (P <0.05, P <0.005). There was statistically significant difference in the levels of CD11b (95.6%, 80.2%), IL-6 (24.0,71.9) and hs-CRP (18.2,9.9) between sepsis group and non-sepsis group <0.05). The levels of CD11b (98.9%, 89.4%), IL-6 (33.1,78.7) and hs-CRP (18.4,11.9) in children with and without sepsis were statistically significant (P <0.05). Results Severe prolonged duration of thrombocytopenia and thrombocytopenia associated with sepsis in patients with RV enteritis and significant increases in the levels of CD11b, IL-6 and hs-CRP could be used as predictors of sepsis, A certain meaning.
