目的:评估双能量动脉、门静脉、延迟CT三期对比增强扫描的碘叠加值与胃腺癌病理分级间的关系。方法:80例胃癌患者均在术前行胃部双能量CT动脉、门静脉、延迟三期对比增强扫描,通过双能量后处理软件得到病灶的三期碘叠加值,并与主动脉进行标准化,计算三期间标准化碘叠加值差值。将每例患者所获得的所有参数与其肿瘤病理分级进行对照分析。结果:具有不同病理分级的患者,其延迟期标准化碘叠加值、标准化延迟-动脉期碘叠加值差值及标准化延迟-门静脉期碘叠加值差值均不同,且差异有统计学意义(P=0.018、0.008、0.047)。多期双能量扫描的总有效剂量为(14.01±4.00)mSv。结论:双能量CT的碘叠加值可作为一种评估进展期胃癌患者预后的新指标。
OBJECTIVE: To evaluate the relationship between the iodine superposition value and the pathological grading of gastric adenocarcinoma in dual-energy artery, portal vein and delayed phase contrast enhanced three-phase CT scan. Methods: Eighty patients with gastric cancer underwent preoperative gastric dual-energy CT arterial and portal vein scanning and contrast-enhanced three-phase contrast-enhanced scanning. The three-phase iodine superposition was obtained by dual-energy post-processing software and normalized to the aorta. Three standard iodine overlay value difference. All parameters obtained from each patient were compared with their tumor pathological grade. Results: In patients with different pathological grades, the values of standardized iodine retardation, normalized retardation - arterial iodine superposition value and normalized delay - portal venous phase iodine superposition value were all different, and the difference was statistically significant (P = 0.018, 0.008, 0.047). The total effective dose of multi-phase dual-energy scan was (14.01 ± 4.00) mSv. Conclusion: The dual-energy CT iodine superposition can be used as a new index to evaluate the prognosis of patients with advanced gastric cancer.
