目的 探讨胃癌局部细胞因子表达的特点 ,为胃癌免疫治疗提供依据。方法 以相应非癌性黏膜组织为对照 ,采用高敏感放射性标记半定量RT PCR技术 ,分析胃癌患者癌组织中纯化的CD+ 4、CD+ 8T细胞亚群及上皮细胞的细胞因子表达谱。结果 胃癌组织与非癌性黏膜组织相比 ,CD+ 4和CD+ 8T细胞亚群中均表现为Th2 型细胞因子表达明显增加 ,Th1型细胞因子表达降低。其中癌组织CD+ 8T细胞的IL 6、IL 8和TNF α的水平显著高于非癌性黏膜组织 (P =0 .0 2 9,P =0 .0 2 2 ,P =0 .0 0 2 ) ;胃癌组织的T细胞及癌性上皮细胞中细胞因子IL 10 ,TGF β1和TNF α的表达较非癌性黏膜组织增高。结论 胃癌局部Th1、Th2 之间的平衡向Th2 漂移 ,提示胃癌局部呈免疫抑制状态。CD+ 8T细胞中细胞因子表达变化更为显著 ,提示CD+ 8T细胞在胃癌局部免疫抑制发生中起主要作用。对不同细胞亚群细胞因子表达状况的了解有助于免疫治疗方案的设计。
Objective To investigate the expression of local cytokines in gastric cancer and provide basis for immunotherapy of gastric cancer. Methods The corresponding non-cancerous mucosa tissues were used as control. Highly sensitive semi-quantitative radio-labeled RT-PCR was used to analyze the cytokine expression profile of purified CD + 4, CD + 8 T cell subsets and epithelial cells in gastric cancer patients. Results Compared with non-cancerous mucosa, the expression of Th2-type cytokines and the expression of Th1-type cytokines were significantly decreased in both CD + 4 and CD + 8T cell subsets. The levels of IL-6, IL-8 and TNF-|Á in CD + 8 T cells in cancer tissues were significantly higher than those in non-cancerous mucosal tissues (P = 0. 029, P = 0.002, P = 0.002) The expressions of IL-10, TGF-β1 and TNF-α in T cells and cancerous epithelial cells in gastric cancer tissues were higher than those in non-cancerous mucosa tissues. Conclusion The balance between Th1 and Th2 in gastric cancer shifts to Th2, which indicates that gastric cancer is locally immunosuppressed. The expression of cytokines in CD + 8T cells was more obvious, suggesting CD + 8T cells play a major role in the local immunosuppression of gastric cancer. Knowledge of the expression of cytokines in different cell subpopulations contributes to the design of immunotherapy protocols.
