目的:探讨两种基因工程乙肝疫苗加强接种的免疫效果。方法:选择244名52~60月龄出生时已全程接种乙肝疫苗的儿童,检测HBsAg、抗-HBc、ALT及抗-HBs滴度,随机分为A、B两组,分别加强接种一针重组酵母基因乙肝疫苗、细胞产基因工程乙肝疫苗,均为5μg,一月后复查抗-HBs滴度。结果:A、B两组加强后执-HBs阳转率分别为96.77%、100%,GMT较加强前分别上升10.71、12.26倍,表明两种基因乙肝疫苗加强免疫效果良好。A、B两组加强前抗-HBs已阴转的儿童加强免疫应答水平显著低于未阴转者(P<0.005)。B组同一等级抗-HBs滴度的儿童加强免疫应答显著优于A组。结论:国产两种基因工程乙肝疫苗于儿童52~60月龄时加强免疫效果可靠,是否表明细胞产乙肝基因疫苗加强免疫效果优于重组酵母乙肝基因疫苗,有待进一步观察。
Objective: To investigate the immune effect of two genetic engineering hepatitis B vaccination. Methods: A total of 244 children aged from 52 to 60 months were enrolled in this study. HBsAg, anti-HBc, ALT and anti-HBs titers were randomly divided into A and B groups, Yeast gene hepatitis B vaccine, cell-derived genetically engineered hepatitis B vaccine, were 5μg, one month after the review anti-HBs titer. Results: The positive rate of HBs positives in A and B groups were 96.77% and 100%, respectively, and the GMTs increased 10.71 and 12.26 times respectively before strengthening, indicating that the two gene hepatitis B vaccines have a good immune response . The levels of boosting immune response in A and B groups that had been overtightened by anti-HBs were significantly lower than those in non-negative ones (P <0.005). Group B, the same level of anti-HBs titer in children with enhanced immune response was significantly better than the A group. CONCLUSION: Two domestic-made hepatitis B vaccines are reliable in boosting immunity at 52- to 60-month-old children. Whether the hepatitis B vaccine is better than that of recombinant yeast hepatitis B vaccine remains to be further observed.
