本文对45例高危新生儿采用含中/长链脂肪乳剂(MCT/LCT)的肠道外营养(totalparenteral nutrition,TPN),并与给予一般支持疗法的30例高危新生儿作对照。结果发现试验组体重尤其是早产儿、极低出生体重儿的体重增速显著、并发症少;试验组与对照组血清胆红素均出现明显下降,且下降速度相近;两组治疗前后碱性磷酸酶的比较及两组高胆儿治疗前后血清胆红素的比较均无显著性差异;两组治疗前后 ALT、AST、SGPT均在正常范围,出院后一个月随访未发现肝功能异常,说明对高危新生儿实施舍MCT/LCT的 TPN能更好地促进生长发育。含MCT/LCT的TPN,对新生儿的肝胆功能影响较小,中/长链脂肪乳剂优于单纯长链脂肪乳剂。
In this study, 45 high risk infants were treated with totalparenteral nutrition (TPN) with medium / long chain fat emulsion (MCT / LCT) and compared with 30 high risk infants who were given general supportive care. The results showed that body weight of test group, especially premature children, very low birth weight children weight gain significant, fewer complications; experimental group and control group serum bilirubin were significantly decreased, and the rate of decline similar to the two groups before and after treatment of alkaline The comparison of phosphatase and serum bilirubin between two groups had no significant difference before and after treatment. The ALT, AST and SGPT before and after treatment in both groups were within the normal range, and no liver dysfunction was found at one month after discharge The implementation of MCT / LCT TPN for high-risk neonates to better promote growth and development. The TPN containing MCT / LCT has little effect on the hepatobiliary function of the newborn, and the medium / long chain fat emulsion is better than the simple long chain fat emulsion.
