目的 :了解化脓性脑膜炎和病毒性脑炎患儿脑脊液 ( CSF)中肿瘤坏死因子( TNF)和降钙素基因相关肽 ( CGRP)水平的改变及临床意义。方法 :采用放射免疫分析法对1 2例病毒性脑炎 ( VE)、1 6例化脓性脑膜炎 ( PM)和 1 1例对照组儿童脑脊液中 TNF和CGRP进行检测。结果 :CSF中 TNF水平分别为 :PM组 0 .85± 0 .2 5 ng/ml,VE组 0 .5 1±0 .2 1 ng/ml,对照组 0 .2 7± 0 .1 1 ng/ml。CSF中 CGRP水平分别为 :PM组 77.1 7± 37.0 0 pg/L,VE组 2 2 .2 5± 2 .8pg/L,对照组 1 6.1 7± 4.9pg/L。PM组 CSF中 TNF和 CGRP水平与VE组比较均为 P<0 .0 5 ,与对照组比较均为 P<0 .0 5。VE组和对照组比较均为 P<0 .0 5。对患儿 CSF中 TNF和 CGRP进行相关性分析 ,结果显示 :PM组 TNF和 CGRP水平成一定正相关 ( r=0 .5 2 6)。VE组 TNF和 CGRP无明显相关性 ( r=0 .486)。结论 :TNF和 CGRP参与了化脓性脑膜炎和病毒性脑炎的炎症过程。 TNF和 CGRP的检测可作为化脓性脑膜炎诊断的一项参考指标
Objective: To investigate the changes and clinical significance of tumor necrosis factor (TNF) and calcitonin gene related peptide (CGRP) in cerebrospinal fluid (CSF) in children with purulent meningitis and viral encephalitis. Methods: The levels of TNF and CGRP in CSF of 12 children with viral encephalitis (VE), 16 cases of purulent meningitis (PM) and 11 controls were detected by radioimmunoassay. Results: The levels of TNF in CSF were 0.85 ± 0.25 ng / ml in PM group, 0.51 ± 0.21 ng / ml in VE group and 0.27 ± 0.11 ng in control group / ml. The levels of CGRP in CSF were 77.1 7 ± 37.0 0 pg / L in PM group, 22.5 ± 2.8 pg / L in VE group and 6.17 ± 4.9 pg / L in control group, respectively. The levels of TNF and CGRP in CSF of PM group were all P <0.05 compared with VE group, P <0.05 compared with control group. VE group and control group were all P <0. Correlation analysis of TNF and CGRP in children with CSF showed that there was a positive correlation between TNF and CGRP levels in PM group (r = 0.526). VE group TNF and CGRP no significant correlation (r = 0.486). Conclusions: TNF and CGRP are involved in the inflammatory processes of purulent meningitis and viral encephalitis. The detection of TNF and CGRP can be used as a reference for the diagnosis of purulent meningitis
