目的:总结青少年眼眶爆裂性骨折的临床特征,评估其手术修复的效果和植入材料的安全性。方法:回顾性病例研究。分析河南省人民医院2015年2月至2018年6月青少年眼眶爆裂性骨折修复手术61例(61只眼)的临床资料。根据骨折缺损面积,其中57例术中植入可吸收材料(PolyMax Rapid),4例术中植入镶嵌钛网高密度多孔聚乙烯种植体(Medpor-Titan)。术后随访6个月,观察临床效果。结果:本组中男52例,女9例。年龄范围10~22岁,平均(16.98±4.45)岁,交通事故为最常见的骨折原因。术前有复视的55例中,术后53例(96.36%)复视完全矫正;术前有眼球运动障碍的52例中,术后50例(96.15%)眼球运动功能恢复正常。所有患者在随访期内均无与眼眶植入物相关的并发症。结论:青少年眼眶爆裂性骨折复视和眼球运动障碍发生率高,使用可吸收材料作为植入物手术效果良好,对于大面积的骨质缺损,可以使用Medpor-Titan进行修复。“,”Objective:To summarize the clinical features of adolescent orbital blow-out fractures and evaluate the efficacy of the repairing surgery and safety of implant materials.Methods:This was a retrospective case study. The clinical data of 61 eyes of 61 cases with adolescent orbital blow-out fractures from Feb. 2015 to Jun. 2018 in Henan Provincial People’s Hospital were analyzed. Based on the fracture defect area, 57 cases were implanted with absorbable materials (PolyMax Rapid) and 4 cases with Medpor-Titan. The follow-up period was 6 months. The clinical efficacy was observed.Results:There were 52 males and 9 females in this group. The ages ranged from 10 to 22 years, with an average of (16.98±4.45) years. Traffic accident was the most common cause of the fractures. There were 55 cases with diplopia before operation. Diplopia was completely corrected in 53 cases (96.36%) after operation. There were 52 cases with ocular motility disorders before operation. Postoperative eye movement function returned to normal in 50 cases (96.15%). None of the patients had complications associated with orbital implants during the follow-up period.Conclusion:The adolescent orbital blow-out fractures come with high incidences of diplopia and ocular motility disorders. The absorbable material has achieved good surgical effects as an implant. For large bone defects, Medpor-Titan can be used for the repairation.
