目的探讨钾通道开放剂烟酰胺对肺源性心脏病患者肺动脉压和血浆内皮素-1(ET-1)的影响。方法78例肺源性心脏病肺动脉高压患者,随机分为烟酰胺治疗组(观察组)40例和对照组38例,分别测定2组患者治疗前、后肺动脉平均压和ET-1的变化。结果治疗前观察组和对照组肺动脉压、ET-1无明显差异(P>0.05);治疗后观察组肺动脉压、ET-1均较治疗前明显降低(P<0.001);对照组肺动脉压、ET-1治疗后与治疗前相比无统计学意义(P>0.05);观察组治疗后肺动脉平均压、ET-1较对照组显著降低(P<0.001)。结论烟酰胺可减少ET-1的分泌和释放,保护内皮细胞,从而降低肺动脉高压,减轻右心负荷,改善临床症状。
Objective To investigate the effects of potassium channel opener nicotinamide on pulmonary arterial pressure and plasma endothelin-1 (ET-1) in patients with pulmonary heart disease. Methods Seventy-eight patients with pulmonary heart disease with pulmonary heart disease were randomly divided into niacinamide treatment group (observation group) and control group (n = 38). The changes of mean pulmonary artery pressure and ET-1 before and after treatment were measured in two groups. Results Before treatment, there was no significant difference in pulmonary artery pressure and ET-1 between the observation group and the control group (P> 0.05). After treatment, the pulmonary artery pressure and ET-1 in the observation group were significantly lower than those before treatment (P <0.001) After ET-1 treatment, there was no significant difference between before and after treatment (P> 0.05). After treatment, mean pulmonary artery pressure and ET-1 in the observation group were significantly lower than those in the control group (P <0.001). Conclusions Niacinamide can reduce the secretion and release of ET-1 and protect the endothelial cells, thereby reducing pulmonary hypertension, reducing right heart load and improving clinical symptoms.
